My favorite bug I can't even tell for sure if it was a real bug, but it didn't happen to me. I just remember seeing the picture from the CoV beta of a Kaiju-esque toon.
Wait, wait.. no, my favorite is getting the initial badges for the Invention tutorial on BOTH sides before it went live. One through the obviously missing ceiling in Steel, and the other through a really tiny crack in Cap that I respec'd my toon into TP just to get the badge even though it involved flying just right or you'd get auto-TP'd away.
After respecing I then got bored with the toon after awhile and deleted it.

I got killed frequently trying to put out so many fires... I knew the building did a lot, but had no idea how extreme!

Also I remember reading long ago something on the forums about a rumour of a time where someone had this 'nerf bat' (or was it a gun?) and they were attacking people with it in Atlas Park & it would make the victims puke?

This was possibly one of the best kept PvP IO farming secret. As you can see I am not on the map at all.

Not a bug (well except that fire/kin trollers were OP & some of them became ego-maniacs), anyhow...
This one time, in Atlas Park, this tard was carrying on like he was god of the game, lord of all fire kins, rulorz of the universe and then brought his ego to the forums and continued to carry on like said 'tosser'.
He inspired me to make a picture of his sheer awesomeness.

And ever since they revamped the RWZ and added all the NPCs & missions there, they had these new map layouts with Rikti decor inside.
And I used to see faces in the decor, and it always used to make me giggle.
The first time I went in one of these maps, I actually wondered if the 'thing' I was seeing would actually stand up and attack me. Then I discovered it was part of the decor. >.>

Back in the fall of 2005, back when The Hollows was a dangerous place, I took my lowbie elec/elec blaster there and was (surprise) promptly trounced. So I clicked on the "Go to Hospital" button. I went to a hospital all right. In Outbreak!
I had to petition to get out, but in the meantime, I defeated the 100 infected to get the badge.

One of my favorite positional jokes is in the second episode of Who Will Die. You fight your way through caverns of Rulu-shin, and if you stand in the right place at the entrance to the final room (with the exit on the other side), the room turns into a smiley face. I have a screenshot somewhere, but damned if I can find it right now.

I have to mention two more bugs, one of the oldest and one of the newest.
A bug that existed from the very beginning was the fact that actually, when we zone into missions, we aren't really "moving" anywhere. We're actually being destroyed and recreated within the instance. That means we are not the same entity when we are inside a mission as when we are outside. And that means powers that "do not stack from same caster" WILL stack if you apply them outside, then go inside, then apply them again. A friend of mine used to love to exploit that bug: it was a ritual for her when running teams to tell everyone to wait outside the door, then she'd buff everyone with force fields, then we'd all go inside, she'd tell everyone to wait at the door for everyone to zone in, and then she'd buff us all again. For a brief period, we were all totally unhittable (and this was long before I7 and base tohit normalization, so that ultrahigh defense meant something). |

The first Bug Hunter was either for the bases (mostly for the ability to keep a hero in a villain zone if there were multiple Atlas Parks open) or for being able to get the character transfer beta to grant players dev access. I submitted those on the same day so it was never clear to me which bug got me Bug Hunter. I was more inclined to believe it was for getting Dev access, because they had already fixed a Dev access bug on beta the day before.
Dev access was granted to anyone that transferred an alt to the second beta server. But they cleared it. The one I found was transferring, in rapid succession, the same alt about 15 times from one server to another triggered some kind of dev/admin rights. I was just trying to see if I could break the transfer system by seeing if it could handle 15 of the same character transfers from one server to another. (I did this because other MMO's stated that you can only transfer an alt once every 30 days, so I figured there had to be something behind that.)
After I logged back into the alt, I had a new, tiny UI window on the screen. Had no idea what it was, so I did a /cmdlist and instead of the normal list that players see, there were a ton of Dev commands. Just for giggles, I tried the /goto command.
Playing my lvl50 MM, and knowing that a friend just created a hero and was in the Outbreak tutorial, I used the "/goto *friendname*" command and instantly I was teleported into Outbreak, as a level 50 villain.
Seeing that I could use /goto to get anywhere, I sent my MM on a tour of Paragon and collected ALL of the hero exploration badges. I was able to turn off collision so i could pass through buildings and war walls. Oh yeah, could also turn "dev" invis on, so was able to sneak past all of the drones undetected.
The last thing I did (which all of this took place in less than 45 minutes) was go into the costume creator and was able to recreate Tyrants costume on my MM. Tinkered around with the back details set, which was funny as some back details were face details.
I logged off and sent ExLibris a messge saying, "Shut down beta, I have dev access and here's some screenies." That was the fastest reply I ever got from her, with "OMG, telling production now!"
Within a few minutes, beta and LIVE was taken down and was down for about 2 hours that night. I remember seeing people post in the General Discussion area with things like "omg, we were given no notice! so unfair! not cool!" I felt bad because they took live down too.
Turns out, Ex mentioned that the transfer tool was linked with live. She didn't get too much into it, but I think the ability was there to queue a "dev" alt from beta to live via the ingame commands.
*Side note about Ouroboros exploit: it took almost 2 years (two years) for them to fix this. I sent this bug in on a weekly basis but no one took it seriously. Every dev that responded with said either "can't reproduce" or "looking into it."
Then when I got Ghost Falcon by the ear, I sent him the exploit and said "been reporting this for 2 years. get a level 25 to 50 in hours. 8 times the mission complete XP, inf., prestige and 8 orange recipes awarded."
The next day, got another email asking what alt I'd like my 2nd Bug Hunter on.
tl;dr:
1.) yes, either I got Bug Hunter #1 for the base exploit or for a Dev access bug
2.) yes, Bug Hunter #2 was from the Ouroboros exploit that could earn one player up to 8 mission complete bonuses (depending on the amount of players on a team.)

Once they fixed that pathing error, they put in a plaque nearby which said something to the effect that the poor NPCs were caught in a mental feedback loop from a Lost psychic, causing them to confusedly run in circles for hours. I always loved that they left mention of it in-game. |

OH, I just remembered an old favorite. In times past, one could form a team and start a mission, and once everyone was inside, you could disband and invite OTHER people from outside the mission. But if you'll indulge me for a bit, I have a story to tell about this...
I once held a "Create your Praetorian counterpart" event for my SG. (Side note: We used to have "field trip" events to other servers, to show others how awesome and friendly we are on Victory. This particular story takes place on the Liberty server.) One day in September 2006, a swarm of twenty or so low-level villains trekked across Mercy Island and into Port Oakes, beating the bejeezus out of everything that crossed their path and led by Null Gulzow-Man*, Devastation X**, Stained Glass Harlot*** and Lord Ouroboros****. They (by which I mean "we") then all piled into a single mission door in batches of 6 or so, much to onlookers' curiosity. What was actually going on is that I, as the mission owner, took the three other characters named above inside, then they manually quit the team. I then invited everyone else in, a few at a time, who in turn disbanded and joined up with the officers above. I think we ended up with three teams of 6 and a team of 5, all inside the "Stone Cold" badge mission. We DESTROYED the Council inside. It was glorious. And everybody involved earned the badge once the mission was completed. Afterwards, we did some more street sweeping and held a costume contest in Pocket D (I actually bought a real-life teeshirt for the contest's winner and had it shipped to him!). For years afterwards, people would bring up that event. It was probably the most fun I've had the entire time playing, which is not a phrase to be taken lightly. That also reminds me of another bug. Well, an exploit I figured out that was pretty crazy that was possible because of a similar team based bug. I never reported it and did test it out to verify it worked so I'll post the details about it before the end as I don't need to get banned now lol

City of Heroes version of Mine Sweeper. I can't remember playing it in another map (other than my own), so maybe I figured it out? Who knows, like all farms it spread and eventually got fixed. You could spawn the arachnos flyer missiles as your base foe group, then a large as possible ambush of level 54 AV's. With a tank, drag them through the mine field and basically nuke them for XP in under a minute. I couldn't help but snicker at the constant explosion of hundreds of those things as you watched AV health plummet. It was even funnier watching some other people try it and getting nuked themselves.
Halloween Trick or Treat mobs not respecting the aggro cap. It brought back the good old days even it it took a while to build up the spawns.

No pics of this one but after the aggro cap was lowered to 17, there was a time where you could still break it with force bubble. I can remember herding up the hollows with it, fun times!
*edit* Oh! In PvP for the short time were blaster melee attacks would detoggle an insane amount of toggles. I can remember hitting stalkers with bone smasher, detoggling their mez protection and stunning them in the same instant, then blazing their face. The cries of "Blasters are OP" were hilarious from the most common vill character in zones who for the longest time had ET with a 50% stun and insane damage and TF with 100% stun and some nice damage. Suddenly blasters weren't the only ones who had to bring BF's to zone.

Way back when the game released the maps were not set up right so you would often see bad guys just leap through a wall. I remember having to petition those a few times.

A horrible bug was coming out of a mission and then fall through the floor. Only re logging cured it.
Happened to me at least 3 times, it was really creepy to see me fall from the map
