Don't know if this has been pointed out yet, but the maverick goggles costume piece sticks out when you use them with the cyborg hat. They stick out of the cyborg hat on the sides.
Bug Report:
When you use the Samurai Chest detail pieces from the vet rewards and a cape, the chest piece crops through the cape in the back on the shoulders. So you can see the top shoulder straps of the armor piece through the cape. This problem occurs with all pieces of the samurai armors.
(Not sure if this was posted before but it is an annoying bug)

Costume Piece: Hair: Ice
When you are made semi-transparent (stealth, shadow fall, etc.), The area beneath your hair but above your eye-line becomes completely transparent.
You cease to have a part of your head. The hair is still there, the area from the eye-line down is there, but the area between is just gone. You can see inside your (empty) skull.
I like the Ice hair a lot, but that bugs me for some reason.
The jet boot's FX seem grossly out of sync with where they should be, the jet FX almost seem to be inside the boot now, instead of coming out of the nozzles of the boot. This only started occurring after i16 as far as I could tell.
This is on a huge body type, editing the costume didn't seem to initiate or fix this issue.

When using trenchcoats on the female model, only the original Dress Shirt (with the tie included) is available as a Chest option. The bow tie that usually goes with Dress Shirt 2 is available in Shoulders for trenchcoats though. It only seems logical that Dress Shirt 2 would be available to use with trenchcoats too.

On the male body, it looks fine, since presumably, that's what the costume was designed for. But on a female body, the line under the male pectoral muscle is now layered on top of the breasts which looks just weird. If Jay, in his infinite grace decides to go back and revise some of the costumes, perhaps the line could be removed?
Oh, Jay is sexy, but everyone knows that. :3

I think the cyborg general's shoulders for female body types are hooked onto the wrong part of the body. Not visible when the character is standing upright, but as soon as a stance change occurs, it's fairly obvious. I colored the arms and shoulders way outta whack to help illustrate.
This is visible in-game in the character creator, tailors, and in normal gameplay fighting NPCs.

If you combine a hat with the full mask option, you have hair.
The Reaper mouth covering mask dissolves into any sort of jacket from the rear.
Something weird at the tailor's. Not sure how to reproduce it. I believe this happened during a Rikti invasion in the zone.
A blank selection comes up and stays on top of all other buttons on the left at the tailor's. If you click in it, a selection occurs and you can no longer change that piece of costume in any way. This happened on one of my characters but it affects two. The chest detail graphic is locked. You can change it and it looks changed at the tailor's and on the login screen. However, in-game the graphic will remain the original bugged selection.
It doesn't seem to affect new characters. I've petitioned it in game and out but nobody wants to touch it or correct course of action.

Bug:
Wolf Spider armor on my female Soldier of Arachnos is missing the top half of the "Spider" back detail. I believe the costume part is
Wolf Spider Collar 1
Wolf Spider Collar 2
Both are completely missing, and attempting to change the collar does nothing at all other than charge infamy.
This is with the Martial Arts pack applied, and it just happened today.
Some Martial Arts booster issues:
* The grip texture on the Chinese Jian appears to be missing or bugged; the grip appears lightless black irrespective of weapon color settings. Either a cord-wrapped (I think this is what the colorizable grip texture on the Chinese Dao is supposed to be), or a wooden grip would seem to be typical, but even the standard rough leather-wrapped grip found on many other broadsword models would probably be acceptable. Ideal resolution: implement the colorizable cord-wrap grip from the Chinese Dao on the Chinese Jian. (Tested on Female Broadsword.)
* Using the Terra or Terra Fire robe, there is no matching Terra / Terra Fire sleeves; and the existing sleeves (including Warrior and Warrior Fire) have a gap at the shoulder join. Ideal resolution: move the underlayer texture of Terra and Terra fire a bit further onto the shoulders, as in the correctly working Warrior and Warrior Fire. (This would insure compatibility with all sleeves; having a specialized Terra & Terra Fire sleeve would be less optimal but still better.) (Tested on Female, several characters.)
* I'm not sure if this qualifies as a costume or animation issue, but Ninja Running with the one of longer bows out on at least some body shapes ends up with the hand in an awkward-at-best position, and the end of the bow stuck up the character's nose. (Tested on Female, Archery)

I noticed this after the last patch, since I was cycling through costume parts trying to find all the new Ninja options. As of the most recent live patch, the Sinister Shoulder plates are displaced about a foot behind the female body. They still appear to animate properly, they're just offset so that they're no longer attached to the character. Additionally, one appears to be higher than the other, so that even if they were attached to the shoulders, one would be about halfway down the character's arm.
I'm not sure if this qualifies as a but or if this is the right way to say this but.

The Tai --> Tai Skin gloves option needs a serious overhaul.
It makes the character look like her arms were stuck in a microwave oven set on high. If that is supposed to be skin, shouldn't it be smooth instead of all wrinkled and ugly? in addition to the radiation burns, it also looks like the skintone does not quite match the skin on the upper arms anyway.

Jay, we know you'll get to the bottom of this. And even though I've never met you, I can tell by your text that you must be sexy.

Also, the 'Braided Buns' detail that comes with the ninja pack does not appear to scale with the sliders, cranium sliders specifically. I was tweaking the head on my character when I notice that my cranium was set a bit tall, but the hair never changed, so my gal has a chunk of bare head sticking out. Like a bald spot, lol.
Edit: Also, the 'warrior full' head detail tends to clip through the body on the female model, or at least it does that in combat stance.

Female tight pants lower body option doesn't have access to the new boots (neither warrior nor tai).
I assume his is an oversight as it's pretty clearly not a clipping issue (tai boots clip far worse with the tucked pants than they would with tight pants)

The hex "mesh" texture on Tech Wings (Male variant, at least) doesn't seem to load properly. It appears extremely low-res and lacks its shader completely as though it has either been only partially loaded or as though it is being viewed from very, very far away. I have seen it load once so far, so I know there's a better texture for it.
