My little sister once named a character named "Dark Godess" who was an Empath/Rad defender with a MUTANT origin.
I also have a warshade named GIGA BREAKER LOBSTER, whose character concept could pretty much be summed up as "every crappy anime cliche in the book thrown into a blender"

My sister has a character (dark/dark scrapper, IIRC, but possibly a dark/dark def) ironically called "Miss Darkity Dark Dark".
Shorter and possible in the character naming space.
She hasn't played her in probably 3 years or so, though.

Oh, that's an easy one- I've seen a lot of racist, suggestive, or just plain bad names over the last 6 years, but the one that still sticks in my mind is the time I saw "Butt Rapin' Billy" in Outbreak. I'm pretty sure he didnt make it out of the tutorial before getting generic'd
The worst names I've ever seen here are not repeatable. Some were racist, others garden-variety obscenity, all obviously designed to pass the filter. I've petitioned a few.
The worst I've seen that I can repeat is "Mutant Blaster". No, he wasn't a Natural Controller, that at least would have some weak irony.

Can't think of any particularly horrible ones off the top of my head. Most I've seen are just generically bad.
That said, I have a Fire/WP Scrapper named 'Generic Shonen Hero' and SS/Inv Brute named 'Manly McMuscleton'.

I've got a elec/elec Brute named Rouge Angle. His bio is
Red, the color of anger. Angle, a pointy thing that can really hurt if it pokes you. Or look awesome if that's the way you style your hair. And don't ask me where these wings come from. Being a mutant really sucks, except when I'm cooking things with my Fearsome Fists of .... um, what starts with F but means electricity or lightning or something like that?
And I've also got a dark/dark defender named Lord Darkity Dark. About the only time I play him any more is when I see somebody make a snarky comment somebody being all darkity dark.

I made a Captain America ripoff right when Shield Defense came out. He was an MA/Shield scrapper, with the costume as close as I could get to Cap's in the costume creator.
I zoned in to Atlas and within 5 minutes some guy runs up and says to me "Hey, that's pretty good, you should see my Iron Man!"
Oh yeah....my character's name?
