You want to do Dr. Creed's story arc (level 5-8) about the infected.
Create a new villain, pick Matthew Burke as your contact, when you finish his stuff Burke will pass you on to Creed. Or you can read about it in the Wiki: Dr. Creed -

Fixing files should hopefully take care of that, delete Coh.checksum first, then run the launcher and tell it to Repair (right click on City of Heroes in the launcher and you'll see a pop-up menu, choose Repair). -

You're summoning pets but not bothering to enhance their accuracy and damage? Why bother with them then.
You're not taking Surveillance or Venom Grenade (OK the redraw sucks but they are GREAT powers).
You didn't bother slotting a Steadfast Resistance/+3%Defense?
If you slot 2 LotG and 2 GotA in some of your defense powers you can pick up an extra 2% Recovery. I think you need to improve your Recovery, even with the Cardiac Alpha I think you'll find yourself running out of End quickly.
No TT: Leadership either? Or Wolf Spider Armor (the extra +2 Mag status protection is worth it IMHO, drop a Steadfast knockback or defense IO in it.)
The defense in Combat Jumping is so small that slotting it for defense is not the best use of slots.

A Shield/* tank puts out a good amount of damage for a tank. Against all odds is your Taunt Aura with a very nice helping of +Damage. Couple it with Super Strength* for Rage and go to town.
*I actually HATE Super Strength but I do have a Shield/Stone Melee tank I love. All the foes go "bouncy, bouncy" around me -

You get the 1.125% HP from CI and from LotG, between the two sets you had 8 of that particular bonus. Change the slotting on 3 of your defense powers, instead of 4 LotG, use 2 LotG and 2 GotA for the extra 2% Recovery. The extra 6% Recovery should help.

You're not soft capped to Range or AoE (Mask Presence's defense suppresses in combat, you're only getting 3.3% out of it with your current slotting). Just adding Combat Jumping will get you just to 45% Ranged Defense. I'd say you took too many attacks and not enough survivability tools, Aid Self would help you there.
End Rec of 4/sec sounds really great, but in an AV fight you're still going to run out of End pretty quickly. NW attacks really suck the End down quick. Taking the Cardiac Alpha would be a better choice for AV fights.
You have 6 of the 2.5% Rec buffs, check Window --> Sets & Bonuses in Mids to see if you're over the cap on any of the IO bonuses. -
I have between 200% and 275% Regen on my defense scrappers, Shield and SR. I would say you could easily do more.

Mind Link makes it real easy, go for sets that give you recharge (and +End, and +Rec, you'll need it with all the recharge lol) and 5 or 6 slot Mind Link with Adjusted Targeting Recharge IO and several Def/Recharge IOs.

You have the Gaussians Build up in Follow Up, so the +100% Dam from that proc is throwing off your calculations. If you want to do the DPS calculation with Follow Up double stacked, remove the proc from the power (leave that slot empty). You'll get the correct damage from the attacks that way.

Jacktar was talking about respecs, some nice tools to help you with that are Mids Hero Designer and Titan Sentinal. Both of those programs can be found here. There's also a link to Paragon Wiki on that page.
Titan Sentinal is a character tracker but it has the added bonus of being able to port your current build into Mids. You export the build using Titan Sentinal, start up Mids, then import the build.
Now you can plan any respec you want to do using Mids; and use the In-Game Respec Helper (under Window in Mids) when you're acyually doing the respec in game so you won't make any mistakes. -

I work retail, so I missed the entire event

Can I get an after action report Hawk? -
The only one one I'm sometimes skipping is a 41-50 villain tip, I don't remember its name off hand. Its the one where you invade a Crey lab, and have to defeat Miss Thistle (and alts) a total of SIX times. She hits Unstoppable and you just have to wait it out, SIX times. I skip it not because its difficult, only because it gets tedious.
