If you can change three things about this game...
3 simple changes that would make the game infinitely more "fun", IMO.
1. All attacks against destroyable mission object(ive)s become auto-hit. Ever punched a wall or other stationary object and missed? Similarly, a Stalker attacking with Assassin's Strike while in Hide should auto-hit (/stalker rant).
2. All attacks against enemies ≥X levels lower than yours are auto-hit. As a balance factor, all attacks against enemies ≥X level higher than yours are auto-miss. The idea that you can MISS an enemy 25 levels lower than you is rediculous.
3. Not "simple" per se, but currency consolidation would allow people to play whatever they want instead of doing only X task for Y reward.
3 long-term changes that the game needs, IMO.
1. New graphics engine, primarily character models.
2. Move away from the outdated D&D d20 mechanics. There are better systems for rolling to-hit values these days.
3. Revamp the Hero and Villain tutorials, level 1-15 contacts, and the structure of Paragon City. Make it feel and look more like a real city, instead of a haphazard collection of zones.
Three simple changes. Here goes:
1) The policy of leaving things half-finished, abandoned in a sub-par state, or just plain broken to go chase the new shiny. See: Power Customization, AE, PvP 2.0, Bases, MM pet AI. This also includes Positron's (I think) statement that they'd rather add new zones than revamp old ones.
2) The philosophy that prioritizes fancy mechanics over solid world-building in story arcs and TFs.
3) The steady and ongoing dumbing-down of the game. As far as rewards go, it's become a matter of jumping through the right hoops enough times. As to newer content, even people who like it have pointed out gaping plot holes.

Speaking of Praetoria and older NPC models, I've noticed that most (all?) of the female NPCs in Praetoria seem to have been redone but the men are still the same classic block-figures. What's up with that?

I'll have to admit that they have fixed many of my main gripes about CoH: the train system goes to all places, no more blue and yellow lines. the ferries take you to almost all CoV zones. The initial contacts and missions have been moved closer so you aren't always crossing huge, danger filled zones with your lowbee. They have included an e-mail system and made "inf" work for everyone. All in all, a lot of good stuff.
1. I'd make all zones accessible to all alignments, but have a new "bounty" system that allows for some PvP in the normal zones if someone crosses over into hostile territory. So, say I take my villian into Paragon City, an arrest warrent could be broadcast by the system and heroes and vigilantes could try to take me out, and vis versa. That may be a pain, but it could be fun.
2. I'll argue that the costume creator needs to be revamped. There are console video games that I played years ago that put our costume creator to shame. I could do whatever I wanted with my character's appearances 10 years ago in some of my PS2 games, and find myself limited in what I can do here in 2011.
3. Undo the changes made to the PvP system awhile back. I used to PvP, I wasn't great at it, but I held my own at times and had fun doing it; I tried it a couple of times with the new system when it first changed and I never went back.

1) Have an issue that is mainly bug fixes of the small annoying stuff. Starting with the Wiki bug list would be a good start but I have a long list of my own I'd contribute (including the female shield-user's taunt bug and the base rental due date bug). A CC revamp with a suite of new simple costume designs would be a good focal for this bug-fix issue.
2) Revamp the contact system. Villain side contacts should be as forthcoming with introductions, even outside their chains, as hero-side contacts. Every bank mission should offer a valid contact for your level! No out-leveling an island and getting nothing, no more getting the level 30-34 contact when you are 35, etc. Make contacts team friendly so everyone can be introduced to the contact by the team leader and follow along co-operatively completing the arcs for the bonus experience and merits. Note that some arcs will have to have their mission order normalized (I'm looking at you Ms Peebles).
3) I like many of what I saw above such as improved MM pet controls (stand and shoot command especially) and the NPC model revamp. I'd also like more attention to the sub-systems such as AE and bases and more diversification of the Incarnate content including contacts specifically for those characters. I'm undecided on which if any of these I'd take over the other. So much is possible and needs doing to make the game even better than it already is.
So if animated breasts are the last thing they should do, at least that means they are on the list.

Graphical and content revamps have already been mentioned many times, so I'll just add: Revamp status effects. It's too binary, you usually have a gaping hole or total immunity. I don't mind being knocked back sometimes, it's kind of cool.
I wouldn't be surprised if we got both at the same time, really. It's both a physics based deforming on the player character model. If they can do it for hair, they can do it for breasts. So if the character models are being updated anyway, then it makes sense to add both at the same time.

1. Revamp all the legacy content and TFs with a heavy editing out of redundant missions, FedEx missions, and kill-all missions. This includes having just one contact give out the arc and not three random contacts (so that players can easily cooperatively auto-complete the arcs together). Move all their non-arc one-off missions to the scanners.
2. Finish power customization with all the pools.
3. Revamp the costume creator so it's more logical. Each piece should have the same set of texture overlays (cloth, rough leather, shiny leather, spandex, flat metallic, high shine metallic).

3. Tab key - Pressing the Tab key targets the next furthest opponent. To my frustration, this often results in me targeting an idle mob in a different group on the edge of my screen when what I really want to do is to target the next nearest opponent who is clubbing me in the face. Pressing the Ctrl and Tab keys simultaneously targets the next nearest opponent (if I recall correctly). But, with my right hand controlling the mouse and my left hand on the keyboard controlling movement via Q, W, E and other keys, it's not easy for me to press Ctrl-Tab during combat, not without looking away from the screen to the keyboard. I imagine there is a way I could reprogram the Tab key to target the next nearest target; but, I really don't want to have to do so. I'd much prefer the standard Tab function targets the next nearest opponent.

1.Make Accolades less grindy:
Seeing as you have to get these several times over if you PvP or are a completist, just a massive timesink that I'd love to avoid.
2.More outside content:
New players complain the game is deserted becaue they can't see anyone. Adding interactive outside content might change this.
3.Better team forming:
Something that would add you to a team or offer the option would be fantastic...

Randomly, I think I'd like to point out that I actually agree with Golden Girl on the lack of jiggle physics for breasts. Now playing Guild Wars I've learned that JPHYX can be done in a tasteful manner that rewards the more observant perverts, but even if it was done on that level, I would still decline such a move for City of, simply because costumes for the classes that do jiggle in Guild Wars are the ones NOT dressed head to toe in stiff leathers or a metric buttload of iron armor but rather given to classes that run around in fabrics, but that is about the ONLY thing they wear.
Meanwhile, in City of Heroes/Villains/Going Rogue/Marcus Cole's Happy Happy Fun Time, a tanker girl can run around in nothing but her undies and bare skin while any squishy girl can don the full plate armors, and anything in between. This is not helped by the fact that certain 'armor' looks can also be found with a 'tights' equivalent, so while I can understand Miss Liberty jiggling with the odd storm kick, how in the bloody hell do we explain why the girl wearing the medieval plate as a tights option because she wanted one of the 3D chest details (which would also create problems with jiggles physics) bouncing in much the same manner?
Now what I would recommend is more realistic breast scales as well as the ability to make female character's flat, or at least a little flatter.
Now back on the top of the thread, three things i could change if I were to rub a magic lamp of the CoH genie would be as follows:
1. Massive Story arc updates and changes to account for the advancement of time in the story, as well as make them a bit more concise, give optional objectives, all those nice things other people have mentioned
2. Massive Environments updates, remove all the excess polygons from the rogue Isles, make the Isles feel like a self-sufficient nation by adding a few more zones, making existing ones larger, add an inter-island highway system and inter-zone transports. Meanwhile, let's spice up Paragon city, we don't HAVE to tear down the war walls, just make them look like they're Alien tech, mush City zones closer together, retcon some of the geography a little to make a bit more sense, have skyway city's highways sprawl throughout the city, oh, and here's an idea, MAKE IT LOOK LIKE A REAL CITY. Even if the Rikti war was, at the start of the game, two years ago, by that time all the rubble should have been organized into modestly neat piles or swept away, or scavenged by local villains.
Since they're environments too, lets have buildings and building/instanced map interiors that make sense! If you need to spice up boring office space interiors, put up barricades that can be destroyed, along with walls and doors that can be destroyed for the times that other things around you can't, have villains bring along portable, commercial grade security turrets and systems, or installed them, or maybe the hellions left some of their ritual and magic trinkets around the warehouse that liven the place up and we can actually see where they sleep.*
Yes, that goes for you too, Praetoria. I'm afraid your shiny, palette and texture swap office labyrinths failed to impress me, and no, putting more posters of Emperor Cole's devilishly handsome face up will NOT sway me one bit. But I will take that offer for the Enriche, thank you.
3. Massive upgrades to Subsystems, such as base building (especially base building), Mission Architect, and PVP. While I understand the Devs want to discourage players from seeing AE as the end-all to gameplay, it should still be treated as an equal of gameplay. Let us drop in cut scene captions, make allies say certain things at certain times, etc. etc.
And because like Lay's 'tater crisps I can't suggest just three
4. Massive revamping of old factions, both graphically, and in terms of power balancing. Because it seems weird that Malta is a spec ops group that stand around in Peregrine in BROAD DAYLIGHT WITH THEIR TITANS PORTING IN and despite all this, they have ZERO snipers, Or that hellions are an arsonist gang and yet only bosses can actually start fires. Hell, I've made whole threads on revamping factions.

1) COMPLETELY re-work all older TFs and SFs (other than Posi). The older hero TFs are just horrid. They do a huge dis-service to this game. They need a revamp. This also holds true of the Shadow Shard TFs. These are a horrid grind and a slap in the face of your loyal customers.
2) Reduce the repetition in mission maps. Make fewer TFs of higher quality. Again, the ITF is popular for many reasons but some of these reasons include unique maps, some unique enemies and a somewhat ok storyline.
3) Do something innovative. We get TFs/SFs, mission arcs and zone invasions - not much else. After 7 years it is time to add some new wrinkles. They took a tiny, tiny step with Going Rogue...time to take that one large step for superherokind! The constant repetition in play style is getting to us vets and will burn people out
(and 4...hehe...allow us to customize other aspects of our toons...the Lore slot would have been a good start but I can't help but feel the ball was dropped here).

1. Sort out the contradictions and plot holes in the back story.
2. Standardise the names of recipes and their respective crafted enhancements. The market search is a complete pita when you're trying to find the enhancement and not the recipe (or vice versa) because on the recipe abreviations are used (but not always) instead of the full name of the atributes (and often those atributes are in a different order between the two), or it's "disorient bonus" on the recipe instead of "chance for disorient" on the enhancement or there's a random extra space in the enhancement name somewhere. It annoys the heck out of me.
3. Remake the tohit and defense system to something more logical for the average player. With a softcap at 100% instead of 45% like now. This has always bugged me a little.
And an extra one:
4. NOT make an underwater zone under any circumstances. In every game I ever played (from Super Mario Bros, over the water temples in Zelda to the sharkpeople city in Baldur's Gate 2) that had a waterzone in any shape or form it was always the most boring part of the game.

It makes more sense for female supers to wear properly supportive undergarments. Even if said undergarments are apparently the size of a postage stamp. Technology in the CoX universe is light years ahead of our own.

As for the three things I'd change...
1) PvP...needs reworked!
2) More costumes! Assymetrical, skin color seperate from costume color so you can put designs on even the with skin options! Different skin types (like skin with tiger stripes)
3) Power changes! Take a cue from another game on the way to do Teleport, Fire and Ice Armor graphics!
