All of this, too:

People with no patience for new/confused players, even when the player is trying to contribute to the team

NPC mez that gives you no possibility of retaliation or escape if you lack a break free
Oh, and when an MM runs around with their pets on aggressive mode and seems to think they're doing more work than the rest of the team.

My two biggest pet peeves are:

1.) People who say "We need a [insert AT here]..." I've seen enough single AT TFs succeed to know that the only thing needed are players that know what they're doing.

2.) People that refuse to invite Kheldians to their team just because they're a Kheldian.

RUDE PEOPLE. By 'rude' I mean people who turn any normal interaction between teammates into conflict.
"That's a terrible build, are you PL'ed?"
"I've never even heard of your SG."
"Need (INSERT BUFF HERE) NOW!"
"You've got a lot of Vet Rewards, but you don't *play* like a Vet."
"I said don't give me (INSERT BUFF HERE), I don't like the way it looks on my toon."

GRIEFERS. This is meant to indicate people who deliberately attempt to disrupt community events, like Hami raids, but it can also mean people who intentionally make PvP unpleasant for others. Also, see Rick Astley.

Are you saying Rick Astley makes PvP unpleasant for others?

People that gripe about how events are ran and rather than just run their own the way they want to they just sit around bitter griping more.

People that are always riding my $%&@, after a while it gets raw and irritated.

This.

But also the opposite - people who rail against every form of control because they never developed a secure ego. Usually that person that joins the team and then warns you what you better not do. See also: Francis.

Oh yeah, and someone mentioned THIS.

Global Channels & PVP events come to mind right away....But maybe I should take this to tells or I may be banned
Interesting you mention this. I find it highly annoying when people ask me 15 times a day if i am going to run kickball then kabitz about how i run it.

Crass ignorance.

'I don't like the look of it at all,' said the King: 'however, it may kiss my hand if it likes.'
'I'd rather not,' the Cat remarked.
'Don't be impertinent,' said the King, 'and don't look at me like that!' He got behind Alice as he spoke.
'A cat may look at a king,' said Alice.

A pet peeve of mine is someone who complains about something but it's ironic in it's own sense. <_<

Yes, it's maddening when people keep telling me I should play professionally.

Burnout; Seriously, I love playing CoH/V, but some days I'm all excited when I log into the game and then my toon sits there and sits there and I can't decide what I want to do because I'm just so burned out and nothing specific seems appealing. I haven't been playing as much in the last year to avoid taking whole months off, and also so that when I do get in game for a static team or to solo for a bit the game seems fresh and exciting again.

Thought ofa couple more.

Insight + Aim + BU + Blazing Bolt
MISSED Death Mage!! Your Blazing Bolt power had a 95.00% chance to hit, you rolled a 96.16.

When the named Boss ofa mission is lower then the all of the other generic Bosses, leadin' up to them.

When the mission is set for +2, +3s appear.
If I wanted to get r4ped by 53s I would have set it to that...

*Enters mission*
Me: "Time to do work!!"
*Spots a mob of 3 minions*
*Takes out 2 of the 3*
3rd Minion: F**k this!! I'm out!
*Minion runs*
3rd Minion: Wait lulz, mebbe I can take him down if I was to turn around and shoot him with mah pistol!! Yea, good idea! Why didn't I think of this earlier!! =D"
*Minion dies*
