Known Issues with IOs
And a collection of recipe names that do not match the crafted enhancements.
These cause a problem when trying to search for both the recipe and the crafted enhancement at the same time to compare prices. For example, I've personally searched for Touch of Death: Chance for NE Damage, seen only recipes for sale, and paid a lot for salvage. After crafting it, looking for the rest of the set, I saw the crafted versions for sale with much lower prices.
Positron's Blast recipes have the apostrophe, Positrons Blast enhancements do not

Devastation: Chance for Hold (Enhancement)
Devastation: Chance to Hold (Recipe)

Note this is also inconsistent with Call of the Sandman
Tempest: Chance for end drain (Recipe)
Temepst: Chance for Endurance Drain (Enhancement)
Touch of Death: Chance for NE Damage (Recipe)
Touch of Death: Negative Energy Damage (Enhancement)
Invention: Stun Duration (Recipe)
Invention: Disorient Duration (Enhancement)
In general, the mix and match use of Disorient and Stun is problematic in the Auction house.
Debilitative Action: Chance for Stun (Recipe)
Debilitative Action: Disorient Bonus (Enhancement)
Pounding Slugfest: Chance to disorient (Recipe)
Poundind Slugfest: Disorient Bonus (Enhancement)
Unspeakable Terror: Chance for Disorient (Recipe)

Executioner's Contract: Disorient Bonus is the name for both the recipe and the enhancement, which is at least self-consistent, but is not consistent with similar recipes. (Yes, the 4 disorient Procs have 4 different recipe names.)
Don't forget these IOs that have discrepancies in the punctuation and capitalization between their auction house entries and the recipe names:

Rope A Dope vs. Rope-a-dope
Multi-Strike vs. Multi-strike
Air Burst vs. Air-Burst
Positrons Blast vs. Positron's Blast

I don't think the search engine in the auction house cares about capitalization, but things like hyphens and spacing are definitely a big deal.
For the blue/white... are you talking about white text in the description, or the white name of the crafted enhancement? (which I think are usually the same, it's just the recipe name that's always wrong, but maybe not) Do you know what the enhancement actually buffs when slotted?
for Touch of Death's chance, I seem to recall it being 15%. So that's worth noting here in case someone comes looking for it. Anyone have a link to where that was said? I agree it should definitely get added to the description in-game.

I think they deliberately use different spelling or phrasing in the recipe and the crafted IO in an attempt to keep people from mistakenly searching for and buying the wrong one. I know, there are like six different "flags" separating them. But I've misbought.
An absolutely tiny annoyance: the Chance for Build Up IO recipe is not correctly capitalized.

Extreme measures, the level 35 IOs can't be slotted at 32, looks like the same bug as the IOs that come into being at 25 not being slottable at 22 as the text says that you can't use this enh till you hit 33.
Had a really good day, my 32 elec/elec blaster fell foul of that and the power that says it takes sleep sets but doesn't (forget which of charged brawl/havoc punch, fixed with I10).

Sting of the Manticore has the same issue. Drove me batty when I discovered this the hard way.

The blue/white text refers to the two description lines in the recipes and enhancements. (It's the same list as I posted a while back here.) I don't have access to the Devastation:Acc/Dam/Rech recipe to check it, but I did craft and slot the Debilitative Action and Trap of the Hunter recipes mentioned here. In every case, the white description line correctly matched the buff given by the IO. So the list should be corrected as follows:
Debilitative Action:Rech/Immob recipe and IO title should be End/Immob
Trap of the Hunter:Acc/Rech recipe and IO blue text should be fixed
Trap of the Hunter:End/Immob recipe and IO blue text should be fixed
Trap of the Hunter:Immob/Range recipe title should be Acc/Immob
One other spelling/punctuation change that should be made is TimeSpace Manipulation vs. Time & Space Manipulation.

Stealth IOs are completely suppressed (useless) in PvP zones, as of Issue 10. Castle has confirmed it here
No one with a brain uses a full or even partial set of Sovereign Right. The Aura IO for this set should be more like the LOTG Recharge IO - it should offer increased damage AND the Aura effect.
At the moment slotting Sovereign right sacrifices a MASSIVE amount of pet damage. The Aura IO replaces the single-effect +42% Damage IO from the normal pet sets. Anyone who thinks this is not stupid should try slotting Sovereign into all their pets and see how crippling it is to MM performance.
I'm fed up with explaining to upset new players that yes they have just wasted a week and 15 million Infamy slotting a full set of Sovereign right because it nerfs the damage of their pets by at least 1/3.

I had basically the OPPOSITE and bugged it. I tried (twice) to craft an ACC/HOLD/RECH and it just gave me ACC/HOLD. Yes, I bought the correct recipe each time... ACC/HOLD/RECH.
And since I already had an ACC/HOLD slotted, it obviously wouldn't let me slot this new one.

The funny part is what you described happening is EXACTLY what you quoted.
I guess I read it wrong. At first, it looked like they were talking about the opposite case.
Don't forget the Uncommon recipies that are requiring rare components to make. One of the Air burst ones do that and I've seen others.
Either make that recipie rare or remove the rare component.
I can't help but think the 2% knockback set enhancement from Air Burst is a decimal off. Knockback in general usually gets twice the buff of other effects from enhancers, so 20% would not seem unreasonable, and might actually have an effect on powers...
Infatum on Virtueverse
All Ice powers (powers with a slow secondary effect) have a bug in text when enhanced with IO set pieces. Refresh buffs are instead displayed as a buff to Slow.
Not a problem with Standard IOs just Sets.

I think that's a problem with standard IOs, and with SO's for that matter. I've seen either that, or something very similar, well before the Invention system.
My memory isn't perfect, so "I'm sure" doesn't mean "I'm right." (Last week I was SURE I'd seen something, I tested it, I was wrong.)

This actually does KnockBACK, and the recipe description says that, but the recipe name is misleading.
Essence of Curare: Acc/Hold (Recipe) all show as Essence of Curare: Acc/Hold/Rech (Recipe). This mixes them in with all the actual Acc/Hold/Rech recipes, and means there is no Acc/Hold listed in the left-hand pane under that recipe set. The recipe description, however, lists it as Acc/Hold, and once crafted it has the correct name.
All Sets that come into existence at level 25 have bugged level 25 IOs. These IOs can not be slotted until you reach level 23, where they should be slottable at level 22. The description does state level 23, which is a bit odd, and not an excuse for this behavior. That affects these sets...

Pet - Blood Mandate
Pet - Sovereign Right
Resist - Titanium Coating
Resist - Aegis
Defense - Red Fortune
Defense - Luck of the Gambler
Luck of the Gambler: Increased Recharge Speed (Recipe) (called Luck of the Gambler: Defense/Increased Recharge Speed once crafted) buffs the Defense of the power its slotted in, and buffs the recharge of all your powers by 7.5%, not just one power as the description says. The recipe description does note the defense buff, despite it being absent from the recipe name.
By comparison, Luck of the Gambler: Def/Rech (Recipe) (called Luck of the Gambler: Defense/Recharge once crafted) buffs the Defense and Recharge Rate of the power its slotted in.
Chance to disorient percentage inconsistencies.
Ten percent from Pounding Slugfest (melee) (this may be intentionally low)
Twenty percent from Executioner's Contract (snipe)
Two percent from Unspeakable Terror (fear) (this seems too low to be right)
Two percent from Debilitative Action (immobilize)
Sometimes you can not replace a slotted IO with a different level version.
The system thinks you're trying to put 2 of the exact same name IO into the same power. There are 2 workarounds:
1) Delete the first IO, then slot the new one. Drag and drop the slotted IO to the trashcan in the lower-left.
2) Move the new IO into the same slot in your enhancement tray as the slot in the power you want to replace. eg, if you are overwriting the 3rd slot in the power, put the IO in the third slot in your tray and then drag it to the power.
What else can people come up with?
