An interesting survey; I do wonder about two juxtaposed questions.
Do you think games are getting more violent
followed by
do you think increased violence is caused by greater graphics
(I forget the actual wording)
how can you answer the second question if you don't agree with the premise of the first?

I would like to add that looking back over my personal 20 years of gaming, although video game violence is more prolific now, the severity of the violence, and adult content atleast in western video games, decreased as gaming became more mainstream, and graphics improved.
Possibly due to coming under greater scrutiny, and possibly due to text being more accepted by censors, and flashing pink blocks just being humurous rather than sexy.

It appears to be biased and badly thought out. There seems to be a desire to push respondents toward the assertion that games are becoming more violent. Control questions are not supposed to be right beside each other so that anyone doing the thing looks at it and goes "that's the same question, phrased differently."
Finally, the data obtained from this is essentially useless since it fails to take into account how people use the games. Example: have I felt strong emotion from playing CoH? Yes and it stayed with me afterward. Why? I roleplay. The emotion comes from interacting with other humans. The survey is really pretty much based on gaming against computer opponents so my correct answers will be interpreted incorrectly.
Bad survey. Most surveys are bad surveys.

Done, but I have to agree with everything that Ravenswing says above.
Yes, I empathise with characters in games, yes, I feel strong emotion when playing games, yes, thos feelings have stayed with me - it is interesting to note that most of the time those emotions are positive ones: Times when I've laughed aloud and couldn't wait to tell my wife about how excited I was while playing.
Further, as another roleplayer, it is not at all uncommon for my emotions to become engaged when playing CoH.
I agree that the questions all seem to point toward a bias on the surveyer, since there is very little questioning positive aspects of emotions in gaming, and I fear that "strong emotion" may solely be taken to mean "strong negative emotion". I have been utterly honest when answering, but I do feel that a scale question "what sort of emotion do you feel most when playing computer games?" ranging from "strongly positive" to "strongly negative" would allow you to tighten the data.

Thank you all for the completed surveys and the feedback. I do understand that he question on what kind of emotion would have added to the questionnaire. I sought to keep it brief in order to not frighten people away. A few more detailed questions would have strengthened the data set and that will be included in my analysis.

My difficulty with the survey was a lack of definition of some of the core terminology. What determines more violence, more killed or more graphically killed? Also having options for games becoming less violent would have been welcome.
Also when we say games are becoming more this, do we mean games in general, or specific games. Because that would have affected my answers in a number of ways. Also when it comes to definitions, you completely left out what is growing to be an integral part of the gaming industry and that is Social Media games.
All in all your survey is for your specific project, and I can't predict what it is you are trying to determine other than that it specifically relates to violence and morality in gaming(and probably the effect of violence and morality in games on the gamer). I just wanted to give some feedback on what I would have liked to see to make the survey clearer to me.

#10 suggests that I cannot feel empathy towards an in-game character without maintaining a distinction between real and game worlds. I saw a "both" answer and chose that, but it specifies "depending on game", as if one game might make me feel, and the other might maintain a distinction, but for some reason you can't have a game that does both.
Any form of fiction can make people feel empathy without blurring the line between real and imaginary worlds. I've felt sad when characters died in various games, or happy when they got to be with the person they loved, but I knew they were fictional.
#12 made me suspicious of the goals of the survey writer. Of course the emotions remain. If people are upset by the death of a character in a movie/game/book, shutting off the movie or game, or closing the book doesn't instantly end those emotions unless you're some kind of robot. This question, combined with question 10, really makes the survey look slanted towards trying to say "see? People can carry their feelings about wanting to kill people in the game into real life!" This may not be your intention, but it makes it look that way.

Just curious: What's your area of study?
As a person interested in market research I read questionnaires with a different viewpoint than a lot of people. Several questions are highly subjective in meaning, and others come across as leading. By the end I really started to feel like there was a bias toward games being too violent and too easy to acquire.
You may want to consider if/how your chosen wording may affect your results when you finalize them. You may want to consider rewording them to be more neutral and less subjective in meaning if you have time and haven't gotten too far in. (Not sure how far you've gotten into data collection. I know that it's non-trivial to reformat a questionnaire and start over.) It will help you get valid results on which you can base your arguments. It will also look better when you defend it.
