Prometheus will open a portal to another parallel world, this particular one unlike any seen before. You'll be deleveled to level 1 and find yourself in the land of Ascalon.
Ascension is when we cross the sea to the Crystal Desert to gain the favor of the gods, beat our doppleganger and Ascend to level 20. At that point we'll be able to take on the Mursaat in the Shiverpeak mountains and finally free Tyria from their Tyranny!

I think it was just introduced because the devs wrote themselves into a hole when they let us be the Most Powerful Ever. They invented a new Most Powerful Ever that they will probs use on up and coming enemies so we can have a challenging enemy to face.

Actually "Ascended" makes sense, since I could really not imagine a universe eating god like Rularuu being tied to something like the Well. Seems it would limit him too much. So thus he's not.

Also, Ascended aren't actually necessary for Incarnates to fight. So far, other (more powerful) Incarnates have been sufficient for Incarnates to fight. And heck, the Battalion aren't Ascended. They're just a race of more Incarnates.

Why Michael is able to censure Prometheus and dictate what he reveals is a mystery for now.
It doesn't really much matter at this point what the original intentions were for Rularuu. Incarnation originally meant that the incarnate was an avatar of one of the gods. It clearly doesn't mean anything of the kind any more. Same with Rularuu. Whatever he started out to be, he's been adapted to the new lore and the question as to whether the Well or Rularuu is of more recent vintage is a mostly meaningless one now.

I have seen nothing to imply that Rularuu was retconned into being an Ascended. Like Scythus said, its more like they just gave a name for entities like him.
