I guess the Devs still decided they rather hide the progress bar until you completed the trial so that people wouldn't be confused(?) about what that half-finished badge was for. Seems like a very weak rationale if you ask me.

Paladin was the sticky one for myself...
I dont think any are rare per se...its just the luck of the draw..the game doesnt know or care that you already have certain ones..its still the same percent chance for each one to spawn no matter if you have run it a million times or once, but run it enough and you will get them.

I have completed MoM 5 times (I think) and failed 3. I currently have 10/16 of the monsters, and I have seen Jack and Lanaruu at least 4 times each. I seem to be missing the Lichen Walker, Jurassik, Bat'Zul, Deathsurge, Caleb, and the Kronos Titan. Most of the people in my SG (that have commented on there progress) also seem to be missing Kronos, Caleb and Deathsurge. Just luck of the draw, IMO, but I know we will get them in time.

Do the Nightmares in the connection breaking phase count? That's where I've seen most of the monsters I was missing. I've seen Kronos, Deathsurge, Lanaruu, one of the other Shadow Shard guys, Avatar of Hami, and Bat'Zul in the first phase.

Although I've encountered some Nightmares twice as often as others, the *end result* (in my experience) is very even. Between them, my two badgers ran MoM (either to completion or initial phase only) 30 times.
Exotica ran MoM 16 times and obtained all 16 Nightmares. Note: She obtained the 16th and final Nightmare on her 16th run, see Code/window below.
Una ran MoM 14 times and sits at 14 of 16 Nightmares. She obtained those 14 Nightmares in her first 10 trials.

Heh in our group last night Caleb was "trolling" us...he was spawning almost every time (ie., once out of the 3 nightmares every time).
The last three that took "forever" to spawn were:
Lichen-Infested War Walker, Crystal Titan and Lunaruu with Lunaruu being the biggest holdout for us.

Well I'll add my two cents to this for what it's worth.
I've managed to get the Dreamwalker badge twice now. It only took one of my alts 12 runs to get the 16 Nightmares but it took the other 24 runs. In fairness the one that took 24 runs had managed to get 15/16 in only roughly 12 runs or so, but it took the rest to get the last one, which in my case was Caleb again. 
My guess is that these Nightmares spawn in a similar way to the groupings of Sig AVs that spawn in RV. They're probably "random" but random in certain groupings like Angry_Angel was implying. This could explain why sometimes you can go dozens of runs without seeing Caleb (for example) and then other times you see him repeatedly.
