I read it as a Survey of things people *want/expect* in an upcoming expansion. Most of the stuff i really don't want to see unless it's a brand new title (CoX2)...a lot of it is very counter to the appeal this game has for me.
'Going Rogue' has been suggested as a potential mechanism since CoV came out Aardy (I'd imagine you're aware of that). I think it was largely rejected in favour of co-op zones. It's popular among badge collectors since it would let them get all the villain badges as well as the hero ones, and I think that's the only real advantage it has.
Edit: And, yes, this was a survey of what one SG would like in an expansion, not a new expansion idea from NCNC.

That's still not confirmed though (when it is, rather than yes it's happening). I think, if it's genuine, that image is the first hint of what the creator will look like. If it is genuine, it certainly seems to hint at allowing story arcs and debunks the "build your own maps from various tilesets" idea.

Definitely looks like NCSoft are testing the waters for a new expansion/new ways to play the game etc. Also suggests that the Mission Creator won't be in Issue 13.

Unless that's a mock-up, that looks massively developed. What's the point in the survey if they've gone that far down the line of production?
The questions quoted in the survey seem a bit... well, obvious. I mean, who doesn't want power customisation? However, adding it would require a pretty extensive engine modification - the CoH2/CO engine perhaps. The new suggested archetype seems a bit odd, presumably aimed at the 'ambigous' starting zone, but there are no details so it's very hard to figure out what that AT would do. The suggested titles and that new AT seem like something that was suggested by players a year or two ago. It all seems a bit odd. A new engine with that kind of content would lock out people who did not buy it in a way which CoV's differentiation did not do (about the only thing you lost not buying CoV was SG bases, hardly a great loss at the time).
Now, given that Cryptic wanted to do another paid expansion after CoV and this was roundly rejected by the tightwads on the US forums ("we pays our subs we don't want a good game by you getting more money off us poor shlubs"), this could be another attempt to sound out a paid expansion given the upcoming CO/DCUO and the possibilities that the playerbase remaining does not contain the people who have left to play games where they have to pay for expansions (which they do happily despite saying they didn't want it in CoX).
But I still think it seems mighty odd.

They may have to be careful with this and keep unlockable content to minor tweaks/touches otherwise it could effectively stop any new mission desgners from getting rewarded. i.e. if the rewards unlock super-special options that help design unique and memorable missions then the first few people to unlock and use them may make everyone elses efforts pale in comparison.
Sure, like most things CoX, they'll manage to pretty-much-balance something out...

A lot of it is obvious. It's not a survey that allows for much beyond what's been asked for (heh, the post on the Champions forums is accusing NCSoft of ripping off all of Cryptic's ideas, which is pure comedy in itself).
It probably is a mock up, but it's included to say, would you pay for this or do you want a lesser version for free maybe.
And given that the minority (ie the forumgoers) killed the paid expansion last time, it's probably a good idea not to speak to them about it again

Maybe why they've gone straight to a sample from SGs - presumably get some 'normal' players and not just the CoX obsessives who would respond to a forum-based survey.

Based on how the game works right now? Capable of jumping between sides, doing standard missions but with Kheldian/VEAT style (no wait, not VEAT, I'd like something more detailed than a couple of missions) story arcs every 5 levels which change dependent on your moral compass.
The AT itself might be the only one capable of swapping between sides on the fly and it's powers will be that of a Stalker/Blaster cross.
That's one theory, anyway.

I believe so. I can't imagine that they'd want to split the playerbase any further after Heroes/Villains, so adding it in as a new AT may be what they're looking at doing now.
Then again, these surveys may come back and say "KEEP THINGS AS THEY ARE DUDES!" and nothing will come of them.

Looks like a survey for a new expansion. Certainly something that NCSoft has put out there. Nothing definite, but very interesting if it comes to pass.
For those too lazy to click on the link, here's some of the things in the survey that NCSoft definitely put out (See post below for link to confirmation).
DOES NOT MEAN IT'S DEFINITELY GOING TO HAPPEN, BUT IT IS BEING CONSIDERED

2.) Characters that do not wish to change sides are rewarded by becoming exemplary heroes or villains, and earning rewards not available any other way
3.) Heroes and villains who go to the other side can also act as Double-Agents
4.) Character Creator 2.0 includes Powers Customization, allowing players to customize not only the character, but also the actual look of its powers
5.) Players undertake missions that define their character's morality and steer them in a more heroic or villainous direction
6.) By switching sides (going from good to bad or bad to good), players can access content "on the other side" (heroes can experience villain zones and content, and villains can experience hero zones and content)
7.) Completely new low-level player experience/zones for new characters in which new characters do not begin a hero or villain, rather become one or the other through early gameplay
8.) New Spy Archetype, Power Sets & Costumes
9.) Universal Enhancement Slots - at level 50, these enhance powers to the character, essentially bringing them to a hypothetical "Level 60" once they have earned all ten slots
