M.I.R.I C. door issues
I had a piece of wall geometry covering a stairwell up to the roof of a building inside a mission (specifically, this one). Had to have a GM move me to the roof manually.
EDIT: Just experienced the exact bug you described. I'm going to /bug it, hopefully I can get a petition through and have a GM fix it.
"You don't lose levels. You don't have equipment to wear out, repair, or lose, or that anyone can steal from you. About the only thing lighter than debt they could do is have an NPC walk by, point and laugh before you can go to the hospital or base." -Memphis_Bill
We will honor the past, and fight to the last, it will be a good way to die...
Okay, wow, these maps are way more screwed up than simple unopenable doors. I had to have a GM move me past one of those doors, and it took him 4 tries. Then when I got inside and started defeating enemies, they ended up getting pulled out into the blocked-off hallway I came from, along with the Seer I rescued.
This needs to be fixed very soon, Praetoria is nigh-unplayable like this.

I've had four door issues now. I'm starting to wonder if new mission maps went live before they were tested.
In the mission given by Transmuter where your Powers Division squad gets ambushed by Reese, the door leading to the roof (and Reese) was blocked by some kind of piece of machinery. No way to get through. Needed a GM to help.
In the mission Steal Data from Neuron's Lab given to me by Dr. Hetzfeld, the server I needed to hack was behind a door that was locked (red bar down the middle) overlaying an open, unlocked door (with the blue bar and outline of the open door just peeking out of the locked door). I have a screenshot of this issue, which I took because the next two door issues are exactly the same.
The third mission with this issue is Get Aaron Walker's Medication given by Aaron Walker. The crate was hidden behind a locked door overlaying an open, unlocked door.
The fourth (and so far, the last) mission with a door issue is the one I just did, Investigate disturbance in Anti-Matter's lab, given by Anti-Matter. I had to ask the GM to wait, because in this mission, I needed to go through a locked door overlayed by an open, unlocked door to talk to Dark Watcher, then go back through the locked/open unlocked door to fight the Longbow ambushes.
I want to thank the four GMs, so far, who have helped me. GMs Brandon, Kalin, Sven, and the patient Zephyr who had to wait for me to parlay with Dark Watcher to phase me through the same door twice.
Devs, I hope you see this thread and take note that your NPCs need to remodel their bases again. I'd be more than happy to email you the screenshot I took if you needed it, but I'm sure you already know what I mean when I talk about a locked door overlaying an open, unlocked door.
I'm also glad to see I'm not the only person having problems navigating some of these tricky doors!
I have the exact same problem with Seer 1381's mission, Finale. I get so far until I come across a flickering door. I click the door and one door opens, but the red door stays. I am unable to go any further and thus, unable to complete the arc. Fix it!

EDIT:
After having a GM assist me with that, I was able to continue mission. One unfortunate caveat is that mission cannot be autocompleted, so it really needs a priority fix if the GMs ever want to get a break.
However, now I'm dealing with the "red-on-green door" issue in the mission "Put Down Cockatrice" for Chimera. Thankfully, I was able to skip it. But others may wish to be wary of mission #3 of Chimera's arc.

Bump and update:
Just ran into my third incident, during the "Steal Neuron's Cyborgs" mission for Dr. Hez-something-or-other in Neutropolis. I've already ticketed one issue and autocompleted another, now I'm just getting angry.
I THINK WE NEED A FIX ASAP.

Agreed, I've hit this bug 3 times in an hour. One auto completed, one gm assisted and I'm now waiting for a gm again. Ugly bug to be sure.
Had this Problem in a Wardog mission
Note: After being teleported INTO the room you'll need to be teleported OUT of the room cuz I had to self destruct to get out of this one. The GM left and neither one of us thought ahead of time on taht one :P

( unsure how to acquire map)
We going to get an "Oh hey this is something we're working on" or are we being shelved? o.0
GM had to finally nuke it when duray teleported out of the room -.- the mobs piled up trying to get to me. this is silly.

I've quit playing in Preatoria until they fix this issue, it makes it next to impossible to run any missions in the lab complexes, and isn't worth the time or effort invested

These door issues aren't just in Praetoria. I'm experiencing these in Primal too. Over the weekend, I'm play those motorized unfolding doors that are usually in pairs(inside/outside) and they make noise like they open but it "shows" closed. Lots of players dying in this process or in the middle. This is a real annoyance.

First gives you map info, second your location in said map, ideal for locating a door issue, especially if you stand facing it when you do both, prior to bugging and requesting GM support, yes I'd do both as it's game breaking.
We are, actually, investigating this. It is a problem with specific doors in specific maps. So one player may see the bug if running Mission X, while another play may not.
The best info for us is the contact name, mission, and the results of /whereami and /loc.

I was on a Lambda trial that had the double door issue once during the acid collection phase.
That Lambda was started on 05-04-2011 23:20:02 Eastern Daylight time on the Guardian server, my character name was Betty Sizzle. I was not on the acid collection team though, but Call Me Awesome was and saw the issue. The time of the encounter with the problem door should be between 23:45:00 and 23:47:00.

No blue doors (after the first) available... will double check... make sure I didn't miss something silly (like a well hidden stairwell)
[Reentered the mission twice; no change.]
[Mission door turned white/lost color]
[Exited to Character Select screen; was forced to relog in; restarted mission... still no change]
[Mission start/entrance is the contact; so I can't even contact him to auto-complete the mission]
Apparently, I play "City of Shakespeare"

Ah, that, yeah. I had to petition twice in one day for that bug, and again he previous day for another bug.
There's a bug in the Transmuter's last mission against Reese, where the door to the roof is blocked by a machine, and the waypoint will point you to what is ostensibly a wall. There's also the very first mission from Aaron Walker, which uses the instance that was pointed out in the pic earlier in the thread. It shows up for quite a few missions.
I reported the second instance and though I forgot to report the first, I hope the responding GM did that.
Samuel_Tow is the only poster that makes me want to punch him in the head more often when I'm agreeing with him than when I'm disagreeing with him.

anyone else having issues with locked and unlocked doors overlaying on each other, happened in two missions in a row, the door was supposed to open, one did, at least the animation opened it, but there was the red door in its place as well, and we could not pass or finish the missions
