are they supposed to be special in any way? or just basically be ultra rare inspirations since they would offer even higher percentages?
Tier 4 Luck - 45% Defense to all
Tier 4 Rage - 75% Damage buff
Tier 4 Insight - 50% ToHit Buff with increased perception range
Tier 4 CaB - full end bar
Tier 4 Rez - 100% HP and END
Tier 4 Respite - 100% HP
Tier 4 Break Free - 3 minute duration?
maybe put in some corresponding debuff resistances where applicable 
The only people who know whether they exist or not are the devs.

actually, they do exist. It is not a rumor. They are not covered by any current Non Disclosure Agreement.
They were first confirmed to exist during the GR beta.
Developers put no restrictions on telling other players about the Tier 4 inspirations as I believe they were still listed in the auction house when GR went into the closing weeks.
Among one of the new Unique Tier 4 inspirations is the Level Shift known as Ultimate, granting a temporary 1 level advancement bonus to the player. What was unknown, and unable to be tested, was whether or not the Tier 4 Level Shift would stack with the known Incarnate Level Shift power, giving a player an effective two level advancement.
The intimation from both Positron and Castle is that players will likely find the Tier 4 inspirations to be useful in the future Incarnate Missions and Task Forces.
So, these tier 4's are not your standard inspirations just with bigger numbers? Sounds interesting. I hope they are rare and have longer durations so I can horde them then sell them.

Without reading any of these post I will just say NO. Half the insp wld hardcap u, even on squishies a tier3 is soft capped.

The soft cap for defense is 45% for all ATs. By "soft" cap we mean that, for most situations and foes, getting more that 45% defense doesn't cause enemies to miss any more often.
The hard cap varies by AT and level, but is generally much higher. So much higher above the soft cap that no one really bothers thinking much about it.

Quite possibly nothing, as they are meant for Incarnate (Level 50, nonexemplar'd) content.
They, as far as I know, never dropped during beta.
