What do you do at end-game in CoX?

No really, I don't do anything.
I either take a break and go play a different MMO/Game... Or I roll an alt.
Why? Because I, personally, have done all there is to do in this game.
I, personally, don't find loads of entertainment in playing something that can no longer advance in any way.
I, personally, love the ding noise.
So at level 50 I get bored really fast... so, for me, it is time to do something else for a while.
Run ITFs/LGTFs/LRSFs with friends.
Host weekly costume contests redside.
Collect badges.
Make new alts.
Play with friends to help them level characters.
Roleplay.
Dispari has more than enough credability, and certainly doesn't need to borrow any from you.

so there is no end-game, is that what I'm hearing.
Is there no epic battle between the heroes and the villains?
There is no "end-game" until you have filled-up all your character slots, ran all those characters up to 50, put purple-IO sets in both builds of each character, ran all the tf/sf, played all the archetypes with the various combination of powers, played all the content in the MA/AE, earned all the badges with all your characters, ran out of ideas for making your own missions in the MA/AE, fought in the arena and various PvP zones, and/or get tired of playing the game.
The "end-game" for me will probably be when the servers are finally shutdown for good.
I don't see that happening any time soon.
So what do I do at "end-game in CoX"?
Throw a big Wake and party like no tomorrow for a great game that has been a joy to play for so many years.
Some might say that's PVP - or the ITF, LGTF, or STF.
The game's in all the levels between 1-50, the various storylines and such. Otherwise, it's a matter of 'perfecting' your character, working on getting rare sets, IOing them out and the like. As someone else put it, "There's no 'end game,' it's *all* game."
Of course, with the upcoming expansion... well, we don't know what it will bring.

Those are what we refer to as Task Forces. The STF (Statesmans' Task Force) etc...

- Mender Silos TF/SF
- Lady Grey TF
- Shadow Shard TFs

- Any leftover content from contacts, including repeatable missions
I don't do TFs/SFs, raids, trials, or PvP myself. My level 50 characters are mostly retired. I play a lot of alts.
Let me help you out with this one Rusty. CoX is not like other MMO's. There is no traditional "endgame" content because CoX is not about the end, like life, it is about the journey. Rushing to the end via powerleveling or what have you makes little sense in this game because that's not where the game happens. It happens along the way. That's also part of the reason this game is a lot more alt oriented than other MMO's.
If you are looking for an epic battle between heroes and villians check out Recluse's Victory, the high level pvp zone. However given recent extremely unpopular changes to pvp mechanics, don't expect to see tons of people there.

the end game is what you make it.
ideally, it serves to keep you entertained.
I don't think of a clearly defined 'end game', the game is just the game to me. Sometimes I log in a 50 and farm, or help friends out with missions. Sometimes I roll up a new character to try out. Sometimes I just log in to mess around on the market.
it's all 'end game', I suppose, after you hit 50 the first time.

Lord Recluse Strike Force/Statesman Task Force/Hamidon Raids to get Hamidon Origin enhancements, they're dual-purpose SOs. Accuracy/Damage at 33% (among others, some affect status effects, movement, etc), enhancing both attributes.
There's also the Imperious Task Force, Lady Grey Task Force, PvP (in a state of flux at the moment), playing with the Markets, unlocking villain VEATs or hero VEATs and the new upcoming 5th Column Task Forces.
Or you can start a new alt to see new powersets...

In a static MMO world, changing the world is difficult. The closest that comes is when finishign a Lady Grey Task Force, the Rikti invade a zone and you can repel them.
Otherwise, you'll have to settle for Statesman/Lord Recluse SFs fighting the top of the top bad guys in the game, or PvP in Recluse's Victory over pillboxes or just smashing people in the face for fun.

You can collect IOs to make your toon totally awesome, then go see what you can do with it.
You can collect badges.
You can join Mothership raids. They always need 50s.
You can go through Ouroboros and get a star next to every story arc.
And if you're still bored, you can make your own missions now. If you're not that creative, plenty of people have made missions for you to try.

Real consequences? As in, you lose, you die permanently?

I'm not sure what you are looking for. Because, no matter what you do, the game must be preserved for the next guy in line to do something cool too.
So if you are looking for an "ending," it's the one you choose to make up. You can say that after you defeat Lord Recluse, he is permanently removed from power, and a slow process of de-eviling begins in the Rogue Isles.
Sounds like a job for the Mission Architect.

I'm going to answer this question: "What do you do at end-game in CoX?" because it seems more open.
I alternate between rolling alts, and perfecting a recent level 50. I say "recent level 50" because I don't go back and touch any level 50 that's been there for a while. My first 50 still has SOs in it, I believe. I'll pull them out if I need one for a specific team, TF, or mission. But other then that, they tend to sit there and collect dust.
Now, my Warshade however, has seen the best end, as I recently fell in love with her again and ended up purpling out several of her powers. I've still got a ways to go on her, and for now she's done in favor of the newer alts (all seven of them now, after a round of deletions).
Of course, a hero's work is never done. Even if you defeat Recluse in his hideous plot to make himself the MOST POWERFUL creature on the planet, the Fifth Column is on the rise again (Issue 15: AnniVersary) , which may or may not be connected to the "Coming Storm" hinted at by the Menders.
And the upcoming Going Rogue seems to be allowing more complex shades of grey in morality.

There's some "hardcore" supergroups out there, where defeat=delete. That's consequences.
