The Hollows wasn't always there. Perez Park used to be the first level-restricted zone characters had access to. Before Hollows, most people ran the mission strings in King's Row.
When the Hollows was first added, only the first row of sandbags existed. Spawns near the entrance were of all levels, happened often in the parking lot across from Wincott, and people fleeing combat would also run groups back there.
During the hours when the area was well-populated, this would lead to huge battles and almost non-stop combat. During less-populated hours, this mostly just led to player obliteration. Enough complaints were generated that the devs added a second line of defense, moved the spawn points away from the entrance, and restricted spawn levels near the entrance to 5-7.
Some of us feel this was a sad day in the Hollows.
I think this is common knowledge, but I feel like I need to include it.
Citadel was first named Bastion.

It was a book called 'The Hammer of The World', and it was the Banished Pantheon. I think.
Break-frees were originally Disciplines, which didn't negate holds, they only added defense against them. For a while you could trade in bounty in SC and get them instead of break-frees.

There was a different "Sister Psyche" that handed out the TF in independence port. Aurora Borealis, who had the mind of the current Sister Psyche trapped inside of her, was the TF contact, and a hero named Malaise was the trainer.
Also, did you know that there was a special TF in the game given by Calvin Scott (Aurora Borealis' husband), that explained how the current Sister Psyche was returned. In the TF, you fought Malaise. That TF was removed with I4.

It was... and they were also responsable for the arrival of the Cabal, Fir Bolg, et al. with the same book, although we didn't know who was responsable for either until we were told that in a Paragon Times article focusing on Croatoa.
Did you know that the tram network originally did not have a location menu? You had to stop at each station between your origin and desitination. Fun.
The various Issue names are the chapter names of various books. (e.g. "A Council of War" is chapter 12 of Treasure Island)

There were multiple villain groups competing over the Malleus Mundi. The Fifth Column figured heavily into things as well.
We mostly got fragments of that story as opposed to a single storyline. I'm not sure I ever saw a single authoritative summary of exactly what happened.

Still, the July 13, 2005 issue of the Paragon Times ties the Banished Panteon directly to the events of the Malleus Mundi with no mention of the others' involvement:

Did you know... that respecification was not always an option in the game? The Terra Volta trial was added in Issue 2. Originally powers and slotting were permanent choices that could not be changed, which meant a mistake in a build was there for life and that patches had much more serious ramifications.

Actually, a lot of evidence points to Crey and/or Nemesis being responsible for the Rularuu invasion. The only events directly attributed to the Malleus Mundi are the Halloween invasion of 2004, and the opening of Croatoa in Salamanca.
(SPOILERS)
In a late game arc, Dr. Quarterfield from Firebase Zulu sends you to investigate several Crey facilities operating within the Shadow Shard. You learn that Crey, using technology first created by Nemesis, has been able to travel to other dimensions (or, at least, just the Shadow Shard) undetected by Portal Corp. We know, already, that Nemesis has been able to contact other dimensions before, including Rikti Earth (being partially responsible for the invasion, himself). So, with that in mind, it's possible that either Nemesis or Crey had left the barrier between Earth and the Shadow Shard weak enough for the Rularuu to invade.
On that note, the Shadow Shard is its own dimension. A pocket dimension, to be precise, created specifically to imprison Rularuu the Ravager. On any alternate Earth, a portal to the Shadow Shard would lead to the exact same pocket dimension as on Primal Earth. So, in all of existence, there is only one Rularuu the Ravager.
The Shadow Shard shares a history with Paragon City. Its natives believe they are, or were, a part of Paragon City from the Sixties. This could mean that the Shadow Shard was created during Primal Earth's 1960s as a prison for Rularuu, emulating our world in a pocket dimension. When Lanaru went mad, he shattered the world of that pocket dimension, destroying anything that could have, once, resembled Paragon City. Or, ruins of what that world once was could be in an as-of-yet locked zone within that dimension (which has been hinted at a few times since Issue One).

Ugh. Those were dark times.
The comic used to be free with the City of Heroes subscription, but that got too expensive.
The group "The Council" replaced an old group, the "5th Column". This was simulated ingame through massive street fights bewen Council and 5th groups. This was the first implementation of NPCs fighting eachother; they didn't used to be able to do this.
At one point, NPCs had their AI for dealing with certain status effects improved to be 'smarter', but this was so drastically penalizing to some archetypes in terms of balance that it was later reverted.
The original strategy guide was essentially based on a design document. So it contains details on a number of events - particularly trials - that were never actually implemented, such as the Faultline Dam trial.
City of Heroes was one of the first MMOs to use a 'early start' program for pre-order accounts.
(Future History!) City of Heroes will be one of the first games to implement the native use of Physics Processor Units.
