Premium Accounts and Free Accounts apparently are not allowed to send or receive PM's. They have limited posting capabilities. They cannot set an avatar as well. It is likely that they cannot set a forum signature.
At least they found a way to allow you to post to get assistance and ask questions. The original idea was that Free and Premium accounts would have no posting capabilities. -
Please post a HiJackThis log so that we can see what else is running on the system.
This sounds very much like a software conflict with another program such as Window Blinds or a similar program from Stardock software. A HiJackThis log will show us what the conflicting program is if that is the issue. -

Very possible. At this point, it seems like /petition is going to be the best way in-game to deal with this. Give them as much info as possible.
You may want to go to the support site at https://support.cityofheroes.com and file a ticket since it allows more text input. Describe in detail what happened, what characters are locked and on what server and as much information as possible to keep from having it go back and forth too much.
If they just aren't getting it and understanding the actual problems after all of the description, ask that it be escalated to a senior GM. -

Just going by past happenings when people were accidentally rewarded Veteran Reward well ahead of time I don't think they'll remove them. They were trying to fix a problem with how/when the VR's were being given and several people were accidentally granted tokens well before they should have received them. We were told they would not be removing them incorrectly granted rewards.
Now, if you really want to be sure, send a PM to the various OCR team members and ask them about it. They likely will have to ask someone else to get an answer (unless someone else has asked them already) but they are good about responding to things like this.
Avatea, Beastyle, Freitag, 7deCoeur and Zwhatshisface (I still won't spell it right, just start typing zwill in the name box and it will show up to select) are the people you want to send PM's to. You can send PM's to multiple people at once, just like an email. -
Hyperstrike, if you've seen WLM's post by now I'll call him to have him change the avatar. He used it since it was handy.

Also, on the login screen down near the bottom, check the box to Link to Existing Account or however it's worded and see if that helps.

TBH, I never realized that the current version of the avatar was so large until I went looking specifically for avatar's that were over the size limits to see if the forums would still accept them.
I almost fell out of my chair when I saw the filesize. -

I didn't assume you hadn't filed a ticket or been working with support. Since you didn't say anything about whether you had filed a ticket or contacted support in any way I didn't assume whether you had or hadn't done so. Instead, I asked a question and then made a statement that was conditional on the answer to that question being yes. Obviously, if the answer was No then it didn't apply to you.
I then went on to give additional information such as the address to the support site which has recently changed. Some people don't know about the change so I find it helpful to point people to the new address.
I also gave a link to a very well written guide for asking for help. Apparently, you didn't bother reading it very well since you think the logs you posted are what's in that guide.
You may want to re-read what I actually said and not assume I said something different.
If you'd like to post the logs requested in the guide, I'll take a look at them to see if anything stands out.
Also, as far as Firewalls go with this game, we've had several cases in the past where people had allowed the game and it's various executables in their firewall and everything was working fine but after an update the file was changed just enough that the firewall no longer considered it to be the same files it had been told to allow. Simply removing and readding the file allowance in the firewall generally resolves things along those lines.
EDIT: Similarly, we've had problems with a couple of anti-virus programs blocking and even removing files, especially after a patch. It may be something worth looking at rather than just outright dismissing it as a possibility. -
Post a CoH Helper report. That will let us see the rest of the graphics settings. We might be able to suggest other graphics tweaks that will help.

Not a problem. All of the Skulls have the Show Player Name option set to Never.

I tested it on an alt account using an avatar I've seen on the forums that WELL exceeds the filesize posted and the 100 x 100 limit.
It's a 193 x 193 image that the forum scales to 100 x 100. The filesize is, and I'm not kidding here, 2,000.83 KB. That's right, 2,048,852 bytes. Think of what people on dial-up experience when it gets to a page with that avatar on it. Sure, there are various ways to block images through the browsers and add-ons, and there is Web Accelerator software that will help with it, but the point is that they shouldn't have to have the forum slow to a crawl due to someone using HUGE files for avatars.
By the way, that avatar belongs to a rather prolific and informative poster so it's highly possible that they may try to open a thread that person posted in, or even one they started. -

Stuff that's already in the system may be "grandfathered" in without the use of addons. Simply because it ALREADY passed a previous check and the board likely doesn't check again.

Something else to remember, if you use the Quick Reply box at the bottom of the page to post a response, you won't see an avatar next to your post until you refresh the page. Using the Quote and Advanced features will show the avatar to you when you post. -

It may be that your new avatar is falling afoul of the size limit.
EDIT: I just checked an icon that I know is over both filesize limits and dimension limits. It's still a huge filesize and it's still being scaled down. Apparently, they haven't set the forum software to enforce those limits, just as they haven't set the software to enforce the limits on Signature images. From what I'm told by moderators of other vBulletin sites I frequent, those can be enforced through vBulletin and/or simple vBulltin add-ons. -
From what I've seen, logging out to the Desktop and then restarting the game usually resolves this. There are times when it's taken more than one attempt of closing it completely and then relaunching to get it to work.

I generally point that description of this forum section out because many posters don't read that part and think that this is the correct place to get official support.
Yes, the Devs do occasionally post here. However, asking for Dev acknowledgement in the first post and then whining in a later post just a bit over 6 hours later that a Dev hasn't responded is generally going to get that type of response along with a link to the support site.
When the poster hasn't said anything about whether they've tried any of the various support functions available to players or provided any Technical information relevant to troubleshooting problems, they'll get the link to the support site and a link to the guide about how to ask for help.
Both are actually helpful responses. Whether the posters follow up with the suggestions is their choice. -
