You might take a look at CoH Splasher.
Using the examples from that site, you can start with this image and you end up with this as your logon.
I've never tried it, so I don't know how flexible it is and the extent to which it can replace images. -

As for Jack, yep, he's would spawn on the throne like that since the beginning. See wiki page. First revision I uploaded back in 2006. Though Chad is right, there was probably a bug fix recently because it has been a while since I've seen him spawn that way.

I'm pretty sure that the Cone Range was straight out replaced with Range and the same with End drain and End recovery being replaced with End Mod.
Wiki Patch notes:
"Existing Endurance Drain and Endurance Recovery enhancements will be converted to the new Endurance Modification enhancements."
So yeah, the separate Endurance Drain and Endurance Recovery no longer exist in the game. I'm pretty sure the same was done to cone range as well.
I don't ever recall seeing seperate team defense and self defense, and I don't think Conceal inspirations ever made it to the live server, just like the Black Market and Wentworth Inspirations.

edit 2: Well, at least one Conceal inspiration ended up on the live server, because there was a sale of one for 1 billion back in March. -

And as far as having enough slots, many powers don't gain any benefit from being 'fully slotted.' To use the MM you posted as an example: the range and recharge enhancements slotted in the the equipment powers are a complete waste.
As for the prestige runs, I wouldn't waste slots on them. Slot one for end reduction, one for speed, one for stealth (if you can afford the enhancement) and if you feel you must, one for jump. You can then switch between them as needed without wasting slots. -

Try starting up the launcher, right clicking on CoH and selecting properties. Then put the same command into the "Extra Command Line Parameters" box.
No guarantee, but that would be my guess. -

First slot, Alpha, talk to Mender Ramiel in Ouroboros. Ask for a portal anywhere it's crowded if you don't have access to Ouroboros.
After you do Mender Ramiel's mini little arc, you'll be set to get Incarnate powers. You'll need to get Notice of the Well eventually from the Weekly Strike Target (a Task Force that changes every week) and the shards you get can be used to craft Alpha slot powers. -
