You seriously need to clean up your text for spelling, grammar and punctuation. A survey is a pretty public affair where presentation matters, and yours is rife with very blatant errors. If English is not your first language, have someone else proof-read it for you, even someone here on the forums.
Beyond that, the survey seems pretty comprehensive, but you have a BIG problem in that it doesn't account for the VIP stipend at all. For instance, I've already spent almost all of 1950 Paragon Points (~$24) on the Market, but not a single one of those points has been paid for by purchase. They are all from my VIP subscription stipend and the original Freedom Launch subsidy for months subscribed prior.
Also, it seems like it's a little early to be asking how often we make microtransactions and give options like "Once every few months" when microtransactions in City of Heroes haven't been out more than... What, three weeks? Also, your options jump from "a few times a week" to "once a month" and there is no in-between option like "a few times a month" or "once a week." You're leaving out a rather large data bracket, which will lead to many people being funnelled into the two brackets around it, and that may not be wholly representative.
Still, I finished the survey and it did take around 3 minutes. I hope you get some useful data out of this.

I am using mostly uniform questionnaire for many other games and not only CoH, hence the other answers.
Grammar is an issue, but It will undergo proofreading before the submission. I am sorry If I offended you by submitting this questionnaire.

As a side note, I do paid surveys occasionally, and it was written about on par with actual surveys for real companies, so I'm not sure about the nit-picking from others. The grammar was a bit wonky in places, but it was readable and not confusing.

I also counted 'micro-transactions' as previous purchase of Booster Packs, not just the new Paragon Market. I hope that hasn't skewed anything.
I will agree with Sam that a questionnaire like that could do with a bit more 'polish'. It felt like a Facebook quiz, which is okay for Facebook.
Does part of the rating of the thesis require submission of the questionnaire itself for evaluation?

Microtransactions are a low financial value transactions,
