I was on a level 3 gravity controller in Talos Island about 3 or 4 years ago. TI was the best place to sell off SOs for cash to fund your toon through the DO levels and after a bit of training any vet can ninja high level zones.
SOs sold at the Science store... it's now time to death port out and two level 27 Tsoo look handy (A sorcerer and an ink man).
Step 1: Run up and brawl the Sorcerer (miss).
Step 2: Both enemies stare at me.
Step 3: Brawl again (miss).
Step 4: WTH?! RUNNN! Dood is crazy reaction.
Step 5: Sorcerer teleports again when I chase.
Step 6: Sorcerer ends up at top of Science store and never moves again.
Step 7: Ink man can't run up the hill to my right.
Step 8: Cast Lift (miss).
Step 9: Ink Man finally makes it up the hill.
Step 10: Cast Lift (HIT!). Ink man ragdolls aaaaaaaall the way down the hill.
Step 11: Ink Man stands up, looks at me, runs back up the hill and never comes back.
Step 12: Run to next mob - Steps 1 to 4 repeat, but after 10 seconds the Sorcerer tps to me - brawls me and kills me.
Still cracks me up to this day.
Thank you, City of Heroes, for giving me a superhero social network combined with amazingly smooth game play. Petitions signed with realistic expectations.

edit: I believe part of the mission info also contains ye old TEXT_MISSING_UNDERSCORE_DESCRIPTION type errors

Had a "WTF did I just do?" moment today. Speeding through a defeat all mission, one of the clockwork ran off and ended up wedged between a wall and some NPC civilian with his hands in the air. Without hesitation, I just shot off a ranged attack. I'm pretty sure it's not heroic to knowingly fire into terrified civilians then laugh aloud to myself but I was in a hurry.

I guess someone had messed with the graphics somehow and made a guy with the bare chested male skin chest detail option, only with a female skeleton so it appeared as if it was a really muscular naked girl with the boob slider all the way up. It was hideous. The guy... girl... thing, made regular appearances in atlas for about a month or so.

Recent WTF moment. I was teamed with a friend of mine doing Level 50 Radios in PI. She selected a new mission, but we couldn't figure out where the door was at first as it wasn't ground level. In fact it was on the upper story of the MA. We ended up not only going inside, but it turned out that the MA portal was the doorway. Needless to say it's been bugged. I don't recall what the mission name was, but it was kinda funny.
Throwing darts at the board to see if something sticks.....

Not the biggest wtf.... but my grav dom has her vday "defeat snaptooth" mish in rv. It's from last year, just kinda funny cause it's in a random door near the hero base. I've petitioned it a couple times then gave up, mainly because I don't care, but it is funny to randomly go into it when being chased by a few heroes.

My biggest WTF moment was when I have accidentally deleted my channels while playing. Still don't know how I did it

I posted some Invention enhancements on WW's for 5 Inf (since I usually lose Inf on this anyway, it doesn't bother me if they sell low), then went to hunt for some badges while I waited for my stuff to sell. Once it sold, I went back and looked at my better then average earnings, then I noticed my 50 Range Enhancement sold for 75 MILLION inf. Either somebody hit too many zeros or the he/she had WAY too much Inf on their hands and wanted to burn some.

It's happened to me as well, but I only got a couple of million.
A few days ago I saw a Crey Agent go to heaven. My scrapper took down the agent and his body crumpled to the floor normally. The targeting box however rose up and through the ceiling then vanished.

PUG WTF MOMENT!
So, I start a Cavern of Trascendence trial and ask for players in Freedom Events, Freedom Monster, Freedom Badge, etc; the regular global channels. Among the people who reply, there's this guy, I'll call him "Pikachu" to protect the guilty.
So, I have 8 people, I explain to them: I'll port each of you to one of 8 obelisks, and we have to click them all simultaneously. After that, I'll port everybody to the final monster. Nobody moves from where I port you to, ok? And a chorus of "ok" comes back, including Pikachu.
I port people to their respective obelisks, do the countdown, and... all the obelisks but Pikachu's are clicked. He's in the middle of the map, battling some Pumicites and promptly getting killed. Annoyed, I port him to my obelisk, tell him to stay there ("ok" comes back) and I move to another obelisk.
I do the countdown again, and... Pikachu doesn't click. I target him, and he's... flying around the middle of the map.
Now quite annoyed, I start blasting him on team chat:
Doctor Leo: Pikachu, WHAT THE HECK are you doing?!
Teammate: is there anyone else you can put on it?
Doctor Leo: no, we need 8
Teammate: damn, that's right
Pikachu is still flying around the map, and I lose my patience:
Doctor Leo: Pikachu, you need to stay at the obelisk and click it when we say so, stop DOING THAT!
And he replies, in team chat:
Pikachu: no se donde estais
INSERT WTF HERE. That's spanish for "I don't know where you are". The guy didn't understand English and was trying to get together with the group. I have no idea how he understood that I was asking for people to do the trial, or how he managed to pick my message among the many messages in English in the global channels I spammed.
So I switch to Spanish:
Doctor Leo: Tenés que mantenerte donde te digo yo
Doctor Leo: Y apretar el obelisco cuando decimos
Teammate: el clicko
Doctor Leo: No te muevas como idiota por todo el mapa
Pikachu: ok
"You have to stay where I port you, and click the obelisk when we say so. Don't move like an idiot all over the map." So I port him, and:
Doctor Leo: Quedate ahi
Doctor Leo: Click in 30 seconds
Doctor Leo: CLICK EN 10 SEGUNDOS
Pikachu: vale
Doctor Leo: CLICK NOW - HAZ CLICK AHORA
Everybody clicks their obelisks, Pikachu included, and the door is unlocked. We rush to Koago, and finish the trial, 13 minutes in. The annoyance past, I relax, and then a teammate hits me with this message:
Teammate: damn, you're pretty much useful for everything, aren't you?
I just had to laugh.

I've had this happen several times, bugged it, begged for a confirmation before delete tab, but nothing's changed. The workaround is to put all your channels in more than one tab; that way, if you delete one tab accidentally, the other tab will still have the channels and so they won't be removed.

Second time was a bunch of DE that were standing a few feet away from the Yellow line in King's Row.
Last time was also in King's Row. A Kheldian zoned into KR via the Yellow Line, and a bunch of high level Nictus spawned inside the Yellow line with her and began engaging everyone in the station.
