That's just really painful looking...I mean really.
Sure some might see it as some form of eroticism but that point on the end of those spines says it tore through flesh to protrude like that. Yeah she's not smiling in pleasure she's trying to hold her uterus in.

Seeing how this spine appears to be growing from the pelvic bone I'd think that there may be another piece of anatomy getting skewered here instead. A very sensitive one. A very painful thing indeed. Hopefully the poison from the spines dull pain.
