Single target Immobilize powers are a matter of contention among most Controllers/Dominators. Many barely use them, some not at all. Heres a small example of 2-slotting:

Theres no real science to slotting flight. Basically you want to max out flight speed. Endurance isnt usually a problem, but heres a way to add some endurance reduction without adding any extra slots:

If you wanted to you could swap out one of the common: Fly IO with a Soaring: Fly IO and take advantage of the minor set bonus.
22 Teleport

Again, theres no real science to slotting Teleport, its just a matter of mixing and matching endurance reduction and range increase. The following example has maxed out range with some endurance reduction:
lvl 35 Jaunt: End/Range
lvl 40 common: Rangex2
--------------------
23% End, 60% Range (56% w/ED).
Similar to the previous example, you can swap out one of the common: Range IOs for a Jaunt: Range IO to get the minor set bonus.

Last Defense IO slotting example has DEF listed twice, with the second instance actually being Endurance reduction.
I think you've done a great job with this. I really like it. Very informative. You don't really mention the additional benefits of slotting with IO's (set bonuses from your recommendations), but even without that it shows you how powerful IO's can be, set bonuses aside.

Of them are location AoE slows, like Caltrops and Tar Patch, and toggle slows like Chilling Embrace and Snowstorm. You can still slot them, but you cant really take advantage of the multi-aspect aspect.
