You get a reward token for every 1200 points, true. Something you need to consider, however, is that bonus points do count towards the 1200. If you haven't kept track of how many points you have actually gotten then it won't simply be "every 1200 points you buy".
I'm not sure about purchasing points in lumps of 400 points if you will get the token once you reach 1200, but you should definitely get one when you purchase the 1200 points outright. Likewise you should get the token immediately, as in applied to your account with the points, when you complete the purchase. I have heard of people needing to log out and then back in for the token to register though. If you still don't see it applied I would definitely contact support.
Please remember that the monthly free bonus point stipend DOESN'T count towards reward tokens. That could easily throw someones numbers off.

Only the points purchased count towards the $15/1200 per reward token.
If your sure you didn't get the reward token contact CS and ask them to look into it.

As several people have mentioned, some people have had to log out and back in to get their Tokens. On my two purchases, the Tokens showed up immediately without having to log.

My fiance's tokens from buying $50 in points showed up almost two whole days later, before we got a response back from billing support (but after the response back from regular support escalating it to billing).
We never actually got a response back from billing since we logged into the knowledge base to close the ticket when we got it. I was kind of miffed about not getting a response from billing before it self-resolved, because there's already so much confusion and terrible accounting from billing in regards to VIP rewards. It wasn't like it was on the weekend, too.

Okay, I have tried to stay out of the "I don't have the right number of tokens thread" The same way I 90% avoid the consignment system in here, except the really simple stuff. I do accounting and auctions for a living, and I desperately do not want those issues in my gaming life.
However.... Last night I bought 1200 Paragon Points so I could get the Street Justice set. Successfully did that, and then checked for my reward token. Nothing.
I have read a couple places that for every 1200 Paragon Points you buy you get one token. THis is not my first buy of Paragon Points, done it (mostly 400 pts at a time) 4 or 5 times. I do not keep track of if I should get a reward token. But, from what i understand when you buy 1200 you should definitely get a token.
Am I misunderstanding the reward system? Is it not every 1200 Paragon Point buy for 1 reward token?
Is there a delay in the reward of this? Next day? End of the Month? End of your billing cycle? Third Tuesday in August except if its sunny, then its the first Wednesday in March? Is it bugged currently?
