Safeguard missions?
First off, on Heroside, always run to the bank as quickly as possible ignoring anything on the streets. That buys some time. But other than that, it's all about kill speed. If you can't finish off the initial door guards fast, then you need to be able to survive both groups at once as you whittle them down.
Get a team or lower the difficulty, because there's really no trick. If you die after engaging the robber or the henchmen with the main robber, then you will probably fail.
I always ignore everything in the front of the bank. Run through, without engaging anything, all the way to the vault. The mobs in the front will stop chasing you and you only have to face the villain and one or two more, depending on your difficulty settings.
I usually beat the bad guy to it and have to wait for them, so they never get started on the vault door. Even when I don't beat them there, attacking them gets them focused on you and you don't have to worry about them getting through, unless you get beat down.
After you beat the ones at the vault door, you have a choice. If you clear the Thief in the front, the timer for the mission starts, and you can add time by stopping the vandals and the side missions. Of, if you don't clear the Thief (the minions there don't matter, only the Lt. with the Thief label) the timer will not start, and you have as much time as you want to clear the map, do the side missions, and get the exploration badge. When you are done doing that, you can go back to the bank, kill the Thief and the mission will complete and vandals will start showing up.

I tried stealthing past the first spawn, but even without a fight and heading straight there the villain breaks down the door before I even reach him, I can kill him at this point but apparently the looters at the entrance still escape (?) and the mission fails again.

1st - Run directly to the bank and enter.
2nd - Ignore the mob in the lobby. The only important NPC's that need to be killed are the ones going for the bank vault.
If you are lucky the guys going for the vault will be at the entrance, if not just run past the lobby mob to the vault. The Lobby mob won't follow you.
3rd - After killing the NPC's that went for the vault, exit the bank.
DON'T KILL THE MOB IN THE LOBBY! IGNORE THEM! KILLING THEM STARTS THE 15 MINUTE TIMER!
4TH - Exit the bank and clear the map and side missions at your leisure.
5th - After you have cleared everything on the map ONLY THEN go back to the bank and finish off the mob in the lobby.
Mission completed successfully.
If you like you can stay in the mission as the timer counts down and fight the final mobs that spawn.
The above steps only work for hero safeguard mission. For villain Mayhem missions the timer starts as soon as they enter the mission.
I've tried: killing the initial looter spawn inside the bank (playing a defender)- while I'm in the middle of this, the villain gets back to the entrance, joins in and crushes me. This sends me to the hospital, with a waypoint to their getaway truck to stop them escaping- except the mission just fails without them coming near it.

The mission shouldn't fail until the villain or one of the looters (easy to spot, they'll be carrying money bags leaking bills) gets to the getaway van. If the mission failed without one of them escaping, something was bugged (did happen to me once).
One minor point. You don't have to rush the bank. The robbery doesn't start until you get within about 80 yds of the bank or until 5 minutes have passed.

Farming It is actually possible to reset the mission, and thus farm a Safeguard mission for badge credits. But some very specific things must be done to make it happen. Safeguards start with a bank robbery that will start automatically even if the bank is left alone. But it is possible to stop the robbery, in progress, without defeating all villains in the bank. The key is the group of villains milling about the bank lobby. They are part of the bank robbery spawn, and must be defeated to complete the mission. But as long as they are not agroed, and the bank vault is not breached, they will never move from their initial spawn point. So the key to making this work is to quickly slip past the group in the lobby and get to the group at the vault. This group should now be defeated before they can breach the vault and activate the villains in the lobby. One major hazard in this is that it is very easy to knock enemies back through the vault door. If this happens, barring the presence of TP Foe, there is no way to get at the enemy now beyond the vault, who will proceed to resume working on destroying the vault door. At this point all players should log out for several minutes to force a hard reset of the mission as soon as possible. If the vault group is successfully defeated, the robbery is halted uncompleted, and the mission will never succeed or fail. Even better, the timer never starts, and the cycle of vandals never start. But the side missions can now be activated and completed, and when all desired side missions are done, the mission can be reset and the process can be repeated. |

I think bypassing the timer just reduces to pressure to hurry. And that way, if you need to pause or take a break in the middle of the mission, you can.
Beating the mission as designed, and beating the mission for purposes of farming are not the same thing.
It's not an exploit, but calling it "as intended" and implying it's the way it is normally done is also inaccurate. Additionally quoting a player wiki and implying that's developer intent is disingenuous at best.
To the Original Poster: Basically you need to be able to beat the looters quickly enough without dying. If you're on a character that gets killed by 1 boss plus 1 minion, or can't defeat the 3 in the lobby before the others get there, look into lowering your settings.
First off try setting it so you don't face Bosses when solo. If that's not enough set their spawned level to -1 of what it would be normally.

I've started a hero for the first time since the game was originally released (left and came back for CoV) and these are confusing the hell out of me, I simply cannot seem to complete one.

Is there some mechanic I'm missing, or is my low damage the trouble? Because the win/fail conditions seem frankly inconsistent with what you're actually supposed to be doing.
