Not sure if lucky is the term I'd use since they are the least effective enhancements, but I don't think they are supposed to happen. I thought TOs stopped dropping shortly after DOs because relevant (around level 15-20) but I don't pay too much attention to all of that since I use mostly IOs and just sell my enhancement drops. 99% certain that they should stop dropping and gone by level 40. Honestly I would recommend bugging it. Sounds like a problem with some changes to the drop system's random generator.

Also, were you running a traditionally low-level content arc with a high-level character? Like an old level 50 doing the Inventions Tutorial?

TOs normally do not drop past 25, yes. Same goes for DOs past... I think it's 35?
Anyway, sometimes contacts will give TOs instead of what they should be giving out. I know Montague Castenella loves to hand out a TO when you finish the Hero Midnighter arc, the cheapskate.

If you were running any old, low-level, arcs; that would explain it.
The arc completion enhancement reward is set as to the type of enhancement it gives, but not the level. So if you were finished an old arc and got the reward drop it would/could be an appropriately leveled TO...

Sorry, but I've gotten TOs, high level, non exemplared - not often, but on occasion, yes. And DOs in the mid-late 30s (I want to say past that, as well, but having just gotten rid of a few I can at least vouch for mid-late 30s.)
It's not common, but it does happen.
I got one from an RSF, the other in my Patron Arc. Not exemplared at anytime. When I say discovered, it is because I did not have the enhancement window open until it turned red, then I said what the... right click delete.

By the way, at lvl 50, a lvl 49 TO gives you a 9% boost to accuracy. I think I'm going to slot Brawl with it.
