Key Binding for enemy selection
There is no way to precisely do what you're asking. The default behavior for [Tab] roughl does what you're asking, but as you've seen, it's not exactly what you're looking for.
Also, FYI: under the default keybinding, [Ctrl]+[Tab] targets the nearest enemy (what you bound Q to, which defaults to turning left)

There is no actual key bind for doing what you ask, or if there is, I've not found it yet.
Personally, I bind "targetenemynear" to q, and "targetenemyfar" to e on all new characters. A combination of hitting q, then using tab generally will go from the closest mob to you, to the ones next to and around him. It's not fool proof, but if you are looking in the right direction, it does a pretty good job usually.
The key is really the camera angle. If you cant see things, the tab button usually wont select them. So if you have lots of mobs close to you, zoom out, and the tab button will often work better when used in conjunction with targetenemynear.

If you go in the keymapping options you'll find that you can map nearest enemy to a key. I generally bind nearest to tab and next enemy to the G key. Works out well.
Another trick I've started using is to keep the targetcustomnext command string copied to clipboard so i can hit Enter, Ctrl+V then just type the name of what i'm looking for. Saves a ton of time.
Actually I keep a text file with all the different binds I use. Copying the one I need to use on a new toon is very quick and simple from it since I most often have it loaded while playing CoH :P
I use "Esc" to clear my current target and then "tab" seems to start with the nearest enemy. Don't do what I do. Ctl-Tab is a better option because "Esc" puts away weapons. I've got five years of bad habits, and I always forget that Ctl-Tab even works.

I bind the `/tilde key... just above the Tab key...to target enemy nearest.
That way its right next to my attacks, and its easy to hit either it, or the tab, with my little finger, while keeping my other fingers over the movement or attack keys.

I've always hated that the keybind for tab was set to target_next so I have always changed mine under Menu > Options > Keymapping.
I set target_near to the tab key, I set the numpad + key to target_far, numpad enter key to target_previous, and I bind target_next to the right mouse button.
My left pinky drops down on the tab key to target the nearest enemy and then I can use the right mouse button to cycle through near targets or I can stretch my right thumb over from the mouse to hit the number pad + key to target far (for things like LRM) and if that's too far away or not quite the right target I use the thumb to hit the numpad enter key to cycle forward a bit.

If you bind keys for target closest, target next, target previous, and target furthest you should be able to target most things fairly quickly without hassle. I like to use Tab, Numpad +, Numpad 0, and Numpad Enter respectively. The only time I have to mouse around is for placeable powers or when there are unusual amounts of enemies like Rikti Raids or some parts of the ITF.
I use targetenemynear$$face or targetenemynear$$follow Those extra commands really help.
On my ranged guys I also have a melee attack set to auto. On occasion my targeting will find something behind me, and I spin, give it a good wack with 1 keystroke. Naturally I choose fast activating attacks for the auto.

I tend to use the two thumb buttons on my mouse for targetnear and targetnext. That's always done a good enough job for me. I don't mind keeping a hand on my mouse though. In fact I prefer it, because I'm constantly adjusting the camera angle so I can get the best angle on what's going on

I keep Tab as is, use Q for nearest target and the tilde key for "follow" (because on my melee archetypes using follow is the fastest way to get into melee range once I have a target). Admittedly in combat I only really use my mouse for mouse-looking (hence why I don't need Q/E as I use A/D for strafing and the mouse to turn), placeables (on some characters), and the side-buttons (for things like Aim/BU), so occasionally using the mouse to click-target doesn't really hold me up much.
Another thing to keep in mind just as an FYI. If you have no target selected "Ranged" attacks will automatically jump to target the nearest baddie when you click the powers fire button. I personally find this particular feature quite nice.
Okay so..starting off, I like to say that my Tab key is pretty retarded. It keep selecting the enemies who are far off the distance sometimes all the way across the map. I have real big trouble keeping "tabs" on close enemies for attacks or debuffs unless I'm actively selecting enemies with my mouse. Which really annoys me since it severely limits my right hand to mouse control where I can be doing things that are much more useful generally like using numpad keybinds and stuff. On my Brute (my only Melee toon) I had to bind my "Q" key with "target_enemy_near" if I wanted to have any chance of selecting a near by mob to attack. Because when the mobs are too packed (and my Tab key being it's retarded self and selecting enemies from the mob that's far away) it's sometimes hard to individually select a baddy to attack with the mouse. Maybe it's just me, but I do have that problem. Using that bind isn't exactly the best solution because then I can't switch between nearby enemies. I can use the custom selection binds like "target_custom_next Cyst" for example, but a bind that can effectively toggle between enemies is something I can't seem to be properly do.
So...if someone can be gracious enough to give me a bind that I can use on my tab key that will work properly? I.e, toggle through enemies, going from nearest to furthest? Because my Tab key is driving me insane.
