Masterminds and Pets when zoning
I haven't played my masterminds since i16 but I heard that all you have to do is buff them with something like a heal and they'll be under your control again. I have also heard about a nasty mayhem MM pet bug but I haven't checked it out myself.

Yeah, apparently, the code to have them zone with you isn't perfect. I came out of a mission, and my lone 'bot was there, all Yellow and unnamed. I couldn't issue commands, or resummon him, but he'd follow me.
I was told that using a buff on him would work, but I was only Level 3, so no buffs to use.
I ended up having to use /releasepets and resummon.

In one case, I had all six bots with me, zoned to Ouro with them, dismissed them, zoned to Cap and had a generic unnamed assault bot follow me into Cap.
I hadn't tried the buffing trick, though, to see if that 'wakes them up'. I have been using /release_pets to clear and reset before summoning them again.
Well it's a new feature that sorta works!

I didn't think to see if a heal would work. I used the thermal shields.

doesn't work with heals, not aoe anyway. I have a nin/pain mm i tried to resolve that with to no avail.
Also, one time i had resummoned the genin and didn't get to upgrade them, but after I completed the mish, exited and the genin were sudden;y upgraded, then went to the next mish and they weren't upgraded anymore. Pretty weird.
Did some testing last night to see what brings the pets back into control. As Beyeajus and others mentioned, no aoe seems to work, as I tried an aoe heal too. Even hit my bots with new forcefield bubbles with no effect.
One thing that did seem to work was hitting a pet with the upgrade power again, and that only brought the pet that you hit with the power back into your control. Another is they do still follow you on defensive, so if you engage mobs they well attack them to defend you and slowly start coming back into control. It usually takes me about 2 - 3 encounters before I have all my pets back.

Known bug along with the SSK auto-defeat of pets when leaving a team mission.
Quickest method to get them back under your control is to re-zone, alternatively getting out of range so they respawn nearer to you also works.
Note: Sometimes when zoning if they are not 'Active' pets, when you get them back they may have lost the upgrades...

I've had the described bug as well. I found buffing the pets restored control.
I did find another oddity, when exemplared to a lower leader, my pets zone with me then promptly all die. This happened with my 35 mastermind when he was exemplared to 31, and to my 50 illusion controller when she was exemplared to 45.
It's not just MM's either. I've had the same problem with the Imps on my fire controller. I'll zone and show no pets in the window, but they will follow me around, and I cannot dismiss them.
Also, something on blue and redside, anybody else notice pets acting weird in your SG/VG base? Not following you or getting stuck on nothing?
They should just roll it back to the way it was before i16. Kinda silly to roll out a half baked feature.
Zoning also seems to (occasionaly) remove the upgrades from pets. And, of course, they still sometimes get trapped by in-mission elevators.

They also seem to like to trade names (well, the lower tier pets do). My medics or arsonists, for example, will not always retain the name I gave them and boast the name of one of their two soldier/thug chums and the other pet runs around with the name for said medic or arsonist. It's annoying.
I had played my MM for a while and saw this on my MM last night. Zoned from Cap Au to Port Oaks, and upon arrival my zombies were NPC's. Entered the Arena, and they were back under my control, but then they did a lap around the arena.
They were both in Defensive Mode.
I didn't have enough time to check on this further, maybe when I get home tonight.
I experienced this earlier today so it's still an issue after more than 3 months. I zoned out of a mission with my one merc soldier and found that he had a yellow name afterward. Later, I'll have to try hitting him with whatever a merc/pain mastermind has at level 4.

had this problem with my bots doing the same, after exiting an ouro story arc mish. I continued to the next mission, where they followed me, still set as rogue bots, then after re-toggling shadowfall the assault bot flicked up back onto my pet info window. As soon as I entered the next mission, they were all back under my command.
Another few glitches I've experienced:
1. During another ouro mission arc, as my Dark Servant deactivated, my assault bot decided to also deactivate - as in he just rocketed out in the middle of a fight
2. When casting Dark Servant, sometimes the upgrade/equip bot animation kicks in on all my bots. This tends to happen more often if I'm surrounded by quite a few mobs.
3. Pets break bodyguard mode more frequently the higher I set my threat level. I usually run at -1/x8 for fast farms, and I occasionally experienced bots running off. Set it to +1/x8 and the increase in bots breaking commands was significant.

Had this happen frequently to me. Healing does work, but it has to be a direct heal, not an AOE. Also had the upgrade disapearing one happen too.
I also reported a minor exploit too, if you have pets die on the way to a mission, or zone crossing, resummoning them before you zone without upgrading them as long as one pet has the upgrade (eg. didn't die) all pets are upgraded after you zone.
Then it's time for them to get off the cross, use the wood to build a bridge, and get over it.

This has been happening to me as well. Also, there are times that the pets, just start Moowalking through the ceiling after they zone into a mission. Bots have been getting stuck exiting elevators too.

I'm going to take a programmer's-educated guess that the bug in the OP is because the pet was re-created before you were fully in-zone. If I'm right then slow-zoners will have it happen a lot more often than those of us that zone quickly. For those directly connected to the servers (like in-house QA) this bug probably never happens.
The exemplar thing happens for the same reason you have to re-accept an examplar with each area change - you become your normal level for a fraction of a second during zoning (perhaps so they can check level requirements for those zones that still require it?). Since you can't have pets more powerful than yourself, the newly created pets immediately die when your exemplar status kicks back in.
But I'm only guessing.

Since I-16 now allows Masterminds to keep pets when zoning there seems to be a bug with it.
This does not happen all the time, but from my experience happens to me after my second mission.
I've notice this when zoning out of a mission. My pets zone out with me but now have gray names, no longer appear in my pet window and cannot be controlled or dismissed. Also trying to resummon pets while the grayed, non-controllable pets are following doesn't work.
It seems after several minutes of either waiting or engaging mobs on the street, the pets will slowly (one at a time) come back into my pet window to be controlled. Also moving to another zone or mission seems to bring them back into control.
This is what I've notice so far and figure I'd post it here. I did a search and didn't find any other bug report related to this issue. If I over looked one, my apologies.
