There are more ways to earn it (see Ouroboros from Paragon Wiki).
The most common is from Doc Deliah in Faultline or Mercedes inside Midnighters. Or simply just get the exploration badge inside Ouroboros.

Actually, the building and spire are the gnomon. The part that people run up to get the badge appears to be a simple support column. Or purely decorative - in a zone with floating buildings, why would they need a support column.

You don't even have to run the mission. Just talk to him, accept the mission, and drop it. Instant Ouro access.

Both sides.
Participate in a flashback started in an SG base. The team leader needs to be level 25 or higher to operate the crystal, the team members have to be at the lowest level or higher.
Effectively both sides can do at any level. (Redside choose one of the intial arcs, blueside the negotiator badge mission)

nothing outside of teaming though? or using any of the flashback missions that you can get the portal before level 25? 
Outside of the 5th column gladiator (can't be gained on any new characters)
The earliest arcs that award the portal and hence the way you can get solo.
Reside: Echo down the Aeons (15-19)
Blueside: Doc Delilah, Agent G or Mercedes Sheldon's 2nd arc (all 20-24/25)
