Manti TF bugged
That's not Manti being bugged. That's a bug that's been reported several times, most often (always?) in TF mode, be it real TFs, flashbacks or, most often, MA missions.
Sadly, there is currently no known workaround.

I'm the eighth person who couldn't rejoin.
When the TF started and we went to enter the first msh, I clicked on the door, appeared to go in, but found myself back outside again. I reclicked to go in, then my system hung up. I rebooted.
When I came back on, it looked like I was the only person on. I had the "Quit TF" btn, but no team. At first I thought I was the only one on and everyone had rebooted. Then I got a team invite by the team leader - eventhough it showed me on a TF. I accepted, and could then see the team.
I clicked to go into the msh and entered - after everyone else had already entered. When I get inside, I'm the only one there. The rest of the team reads "on mission", but they are not with me 0 AND I'm now level 50.
I exit msh, try reentering, to no avail.
I log off and try to log back on. I get an error that says "A problem has been detected with the COH database. Please contact customer support". I quit, restart COH and verify files without incident.
I log back in. It shows me on a TF, but again, no team, eventhough I'm able to private tell them. My msh screen says "Mission complete!". I have no red arrow or door to enter. I go to Manticore, he says "It looks like you have other things to attend to."
I try rebooting my computer and relogging a couple of times to no avail.
I send a petition (and never hear back). After about an hour of wasting time in WW (still with the "Quit TF" btn) waiting on the GM, all of a sudden the "Quit TF" btn goes away and a person has joined my team. The toon is Brainwave and he said he had been on Manti last night but every one quit. Keep in mind, I never invited him, he just magically appeared. We chat for a bit, then he quit the team. The "Quit TF" btn reappears.
Still no word from a GM, but I stay on just to see what happens when the team I had been on completes the TF. Nothing. No merits and the "Quit TF" remains. I wait a little longer for GM, but finally give up and log. I left the "Quit TF" btn. I'll see if it is there when I log back in.
That implies you logged into somebody else's Manti, which is a pretty serious database error.
Most of us were in Europe so it was getting a little late to restart it otherwise we would have done.
Logging into a second instance is not new, I had it on the Waldorf factory map psychic clocks mish a couple of years ago, but only seen it regularly in MA when you add somebody to a team already in a mission.

I was one of the two who ended up on a different mission map. We just went ahead and started killing stuff too, and then the exit mission button didn't work at the end. I just died to get out of it.
It was pretty bad, though. The first two mission door crashes, I had to ctrl-alt-delete CoH and restart, but luckily I didn't have any further problems on the TF. Well, except at the end when the game crashed on exiting the final mission. I thought it was a good capper to the way the TF started. I didn't get notification of any merits, but I'm pretty sure I did get them (can't remember the amount I had beforehand).
Tumbler, look me up too if you want to run it again. I'm sorry you never could get back in sync with us! I'm @Thessalia.

Did the toon name that appeared as this poor guy's doubting his sanity make anyone else gigglesnort?
I still need to do some testing (not a high priority at the moment) but I have a working theory that when any person accepting a team invite has an active MA arc set, they are able to join the team and it "appears" the MA arc has been dropped by said user, when in fact it lingers in the background. When the team sets a door in a non-MA arc like a TF and such, most if not all the members crash entering the door.
Every person on a team needs to physically drop any MA arc they are active with before accepting an invite.
I have run numerous test using 3 accounts and those issues have been easily recreated each time. How wide spread it is I am not sure. I have not tested it a lot with TFs yet.

Has anyone heard anything on this issue? I can't run a TF or A SF for my guild weekly anymore, we ran into a splitting issue and then someone joined back after a disconnect and they were in a TF by themselves o.@ And at another location all together.
Any word? This is crippling if there's a risk of this each time. I was running the psyche tf atm.

This has been happening to me while on an AE team.
I can only assume that this is related to TF mode as one of the posters already suggested.
I have NOT had this happen while regularly teaming or soloing.

The 5 that cleared the first mission also completed it for the 2 on the other version of the map, but their exit button didn't work, after they exited the long way, they could join up with us for mission 2.
The 8th never did get back on the TF.

2nd Edit several of us then DCd on exiting the final mission, so no idea if we got any merits as was no message before or after exiting to say.

Some people on you team were im MA TF mode probably. We had similar issue on 1st or 2nd day of i14 release. We formed ITF with half team still being in MA mode. When entered 1st mission, half of team members just disappeared from team list. Actually they went into different instance of the mission. While exiting it, all of them had hard crash. After they came back, we quit TF, reformed team and restarted ITF without any problems.

Regarding the crash on TF completion: while, IMO, it is unrelated to 1st issue you described, I saw the similar thing 3 times in last week. It happened to some of my teammates during Manticore, Sister Psiche and Citadel TFs. Strangely enough, I have never been affected by this.
So the problem is happening when your in an MA TF-Lite Mode and THEN accept the invite for a SF/TF without quitting your current MA? Making sure I'm covering bases here. Alot of our structure is around TF/SF's and this as I said was crippling.

Yes, it was my point regarding problems on TF start. But I don't know what causes crashes on TF completion (btw I haven't seen such problems for other TFs i ran, like LGTF, STF, ITF)

Yeah, I ran a Sister Psyche TF sometime after Manti last week on the same character, and had the same end-of-TF crash at the end of it, along with at least a few others. We did not have the issues from the Manticore TF on this one, so it does appear the crash at the end is something different.

Yes, I've had that 'crash at the end of the last mission' business a few times. I don't see it as a big deal as it doesn't even mess up your rewards. One crash at the end of a 2-3 hours TF isn't onerous, just a nuisance.
I have not seen any version of the other problems mentioned except in AE arcs - never on an actual task force and I run 3-4 TF's every week.

It's true, the end of TF crash does not mess with your rewards. I run TF's fairly frequently and so far no one has had any issue with rewards concerning the end of tf crash.

Required question: Are you sure you didn't get the merits? I can't count how many times I failed to get the 'Merit Reward' or chat log spam, but got the merits anyway.

Usually, i'm trying to keep track of my merits as well. It is true that sometimes you are getting merits even the merits reward message doesn't hit chat log, but in some cases you just not getting merits at all
