According to Mids, you're only picking up 5 secs on Hasten with that third slot, and without it Hasten would only be down about 3.7 sec. Might consider using that slot elsewhere.
You might consider trading either Hibernate or Frozen Armor for Ice Storm, as it deals lots of damage. If you steal the slots from Spectral Terror, which really doesn't need it, you could slot it with 5 Ragnarok. You'd lose 5% recharge from what you currently have in Spectral Terror, but gain 10% recharge for Rag, for a net gain of 5%, plus a power that deals a lot of damage over a wide area.
I like adding a third damage proc in Freezing Rain - either Positron Blast Chance for Energy Dam or Impeded Swiftness Chance for Smash Dam. It turns FR into a pretty effective AoE power cause something almost always hits. In fact, I have FR, Frost Breath, and Ice storm all 6 slotted, with as many damage procs as I can squeeze in them.
I've played with Mids for hours and I've never quite been able to get a perm PA build, even with 5 purple sets with their 10% bonuses and 5 LOTGs, but I can get within about 2 seconds. I usually just run an hide or fly up really high if outdoors, and wait for PA to recharge. Depends on if Phantasm is cooperating.
I have a Force Feedback Chance for Recharge in Gale. Because of all my high level sets, I have a global +63% accuracy, so I don't miss slotting gale with Acc. If you hit once per minute with it, that 5 sec +100% recharge boost is like another 8.3% gain in recharge per 60s which I think would get 62s PA down pretty darn close to 60s.
I can say that I rarely had endurance problems with my build pre-Incarnate, and now with Cardiac Boost, I never have them. The toggles I usually have running are: SupInv, Hover, SS, Maneuvers, Assault, and Snowstorm. No Hurricane currently. I also always have 2 Lightnings and 2 Tornados out as well, and FR every 17.4 secs. My current PA is at about 65s.
Here is my goal build which has PA at 61.89s, but I haven't tested it for endurance yet. The difference in this and my current build is that my LOTG toggles all have Def/End slots as well as the +7.5%'s, which admittedly does add up. (They also give me 5x10% regen bonus which is really nice. I will hate to lose them when I respec!)

Would love to see an event in Dark Astoria where the fog lifts so you can actually see the zone. Maybe make Adamastor more like the Paladin construction. As the fog lifts, the BP start gathering from all over the zone to begin a ritual to summon Adamastor.
For Boomtown, I would love to see them start rebuilding it near the gate like they did in Faultline.
For more content, they should make some Ouro missions or maybe a time traveling TF where you go back in time to these zones before the Rikti invasion, before the hollowing, etc, to see them in their original form. Would love to see Faultline, DA, Boom, the Hollows, etc all shiny and new. -
I'm the eighth person who couldn't rejoin.
When the TF started and we went to enter the first msh, I clicked on the door, appeared to go in, but found myself back outside again. I reclicked to go in, then my system hung up. I rebooted.
When I came back on, it looked like I was the only person on. I had the "Quit TF" btn, but no team. At first I thought I was the only one on and everyone had rebooted. Then I got a team invite by the team leader - eventhough it showed me on a TF. I accepted, and could then see the team.
I clicked to go into the msh and entered - after everyone else had already entered. When I get inside, I'm the only one there. The rest of the team reads "on mission", but they are not with me 0 AND I'm now level 50.
I exit msh, try reentering, to no avail.
I log off and try to log back on. I get an error that says "A problem has been detected with the COH database. Please contact customer support". I quit, restart COH and verify files without incident.
I log back in. It shows me on a TF, but again, no team, eventhough I'm able to private tell them. My msh screen says "Mission complete!". I have no red arrow or door to enter. I go to Manticore, he says "It looks like you have other things to attend to."
I try rebooting my computer and relogging a couple of times to no avail.
I send a petition (and never hear back). After about an hour of wasting time in WW (still with the "Quit TF" btn) waiting on the GM, all of a sudden the "Quit TF" btn goes away and a person has joined my team. The toon is Brainwave and he said he had been on Manti last night but every one quit. Keep in mind, I never invited him, he just magically appeared. We chat for a bit, then he quit the team. The "Quit TF" btn reappears.
Still no word from a GM, but I stay on just to see what happens when the team I had been on completes the TF. Nothing. No merits and the "Quit TF" remains. I wait a little longer for GM, but finally give up and log. I left the "Quit TF" btn. I'll see if it is there when I log back in. -
Excellent guide. My only exception is that I think tornado should get a higher rating. Of all the builds that can truly get the fullest use of tornado, it is an earth controller. I play my 50 earth/storm two ways:
1. "Lock'em down and pound'em" - Stalag/Cages/Sleet/thunderstorm/tornado/other AoE damage powers...repeat cages as needed. This is how I play most of the time. I slot Tornado dam/dam/dam/end/rech and love to watch it chew away at the baddies in cages.

2. "Keep'em on their butts" - Alternate QS/EQ, FR/SS, and VG and let others blast away - I do this a lot less, but if we already have a lot of damage and I don't need to bring out "Wendy" or "Stormy", it really mitigates team damage. -
This may have already been suggested, but here goes. A small change that would be really nice to TP would be to change the way it behaves when you materialize. I would like it if you simply hovered until you try to move. Allow the toon to spin around so you can target your next TP location, but don't allow him/her to actually travel. If he/she does, then the hover is canceled and you immediately fall to the ground.
This would address the complaints people have about having to take hover with TP just to keep from falling to the ground. It really isn't giving people a free hover because you wouldn't be gaining the ability to move like you can in hover.
Plus, by canceling hover the moment you try to move, it would eliminate that awkward hanging you do when materializing and attempting to run. I know I would certainly like to be able to move the second I materialize and not be forced to hover for a couple of seconds.
