Cone Attacks Always Miss
The main reason it was a bug, not only would you miss, but you wouldn't even get the MISS above your target's head.

Watch your combat tab.
When you activate a power, you get a "activating<power name>" message. Then there is a message that either "<power name> missed" or <You hit with <power name> for xxx.xx amount of damage".
With this bug, you get no message after the activating message. You activated the power. The game used your endurance and you performed the animation, and the power needs to recharge. But the game never even checked to see if you hit or miss.
So it's not a "miss" per say. And if you count your hits vs. your misses, you wont see the problem. Think of it as failed activations of cone attacks.
Sorta like a Shaq shooting a free throw. If the ball hits the floor having passed through the hoop, it's a hit. If the ball hits the floor having NOT passed through the hoop, it's a miss. If the ball disapeers in mid-air, then you have this bug.

Yes, the power would effectively do nothing.
Take a look at this combat log demonstrating the bug: Picture!
Electron Haze is a cone power, and as you can see, I activated it without it doing a thing. There's nothing between the text informing me that I activated it and the text informing me it's recharged.
If you're actually missing (complete with combat log representation), then that's not a bug.
I'm curious if the Ninja skill Golden Dragonfly is considered a cone attack and if it's affected by this bug.
The wording of the skill makes it unclear how it actually works, but it seems to be a "sometimes" cone attack.
This skill is new to me, and has an alarming miss rate. And like others have noted, misses equally against -2 mobs and +2 mobs.

Ok, now this I was able to fully understand, thank you. The subject is a bit missleading IMHO.
this explains a lot for my stalker, but this post has saddened me all these people chatting it up with devs, ive sent and recieved a couple from States and Manticore but thats it im going to lock myself in my room and cry

Hrmmm... I understand that, but I've only seen that happen with ranged cone attacks, not melee cone attacks. Yet Castle explicitly stated he tested MELEE cone attacks to find the bug. Seriously, I've never seen melee cone attacks activate and not do anything.
As for the range ones, The only time I get that is if I click on smething at the very edge of my range and hit the cone right as it moves away. OR sometimes it lets me click when I know the range is just a tad too far. That's the bug your experiencing. It's a latency issue. Your client thinks it's okay to activate the skill, tells the server, I'M USING IT, but the server goes, wiat a second there is nothing there to activate on. So instead of your client not activating, you do and you get nothing for the effect. That's how I've always seen the range bug your talking about.
Let's get this straight, what MR Quizzles is reffering to is not a MISS bug but a failed ACTIVATION bug. Two seperate issues. A MISS bug means it has something to do with accuracy and missing attacks not attacks failing to activate properly.
Thought I should add:
The past 2 nights I have had amazing results with Shadow Maul! I don't know what changed but it seemed like I never missed! I did hit level 30 so that might have done something.
I was racking up kills left and right with SM and air superiority!
Seemed like everytime another bad guy would come close to the one I was attacking I would get them as well.
I must have taken out 4 Rikti monkies with 1 SM!
Just wanted to clarify that my earlier post was just skewed perception.

While I'm in the same boat (I've never seen my attacks always miss), I had just gathered several hours of play data last night that showed that my sonic cone attack with 2 +acc enhancers was indeed missing far more than it should be and was about to send it in.
And then I saw this post. Can't complain when the Devs are already fixing something that I just found out was broken.
