This doesn't require an add-on. Part of the base UI for quite some time now. -
Didn't know you were on all IOs. Most people are by that level, but not everyone. You didn't say.
Still, practically every patch has at least one person swearing up and down that their accuracy was killed by the patch and a hundred others saying theirs is fine. Perceptional bias and an unlucky streak could be all there is to it. -
Regen is functioning as it always has. Accuracy hasn't been changed. Enhancements expired, perhaps?

Well, not everyone obsessively follows every dev post, so it's not unreasonable that they hadn't come across it. On the other had, they could have checked the dev and community digests before asking, which would seem the obvious thing to do.

I first started a CoH Account in 04. I can't seem to find anything to keep me playing beyond level 12 or so.
I get bored.
Is there anything fun to do? Any grouping? I am not sure if I have grouped with anyone, ever. Well maybe once or twice.
When I first started playing, there were no real items or inventions etc.
I run around the streets and encounter the regular thugs, I will do some missions but everything becomes very routine. Is there anything fun to do .. at all?
I love the character creation, who doesn't?
I would love to find something to do.
Help me out.
Another thing I note is that you sound like you street sweep more than running actual missions. That does get very tiresome. Stick to the missions and actually read the stories behind them.
12 is really only just the beginning of things. You've only just graduated out of training enhancements and barely have any of your powers. -
Not likely. Raising the level cap doesn't solve any problems, just puts them off for a very short time. And creates new balance problems...

Actually, what was said was "16th *ISH*". It's still possible that it'll drop before the end of the week, but I'm not going to hold my breath. It absolutely won't drop before Thanksgiving if it doesn't drop by the end of this week, though. They're not going to release an issue Thanksgiving week.

You're at the cap. Flight speed caps lower than other travel powers because it's also the safest and easiest travel power to use. Unlike running, you can make a straight line to your goal in most zones. Unlike Superjump, you almost never hit the ground and never run into any obstacles too tall to go over. And, unlike Teleport, it's "fire and forget" ( No need to "click click click click" to get to your goal ) not to mention far less end intensive. Some disagree with me on this, but if the flight cap were higher, there'd be no reason outside of concept builds to take any travel power other than fly.

In short, everything directly connected with GR ( which includes the Incarnate stuff ) requires GR.

Check the wiki, this behavior has been explained there for ages.Those that work for a limited time say they are limited in duration after using. And they last 120 seconds.
Edit: Minor correction, some special IOs are always on, period. Some are "procs", which trigger as I explained above. The one in question falls into the latter category. They won't necessarily say outright that they're a proc, which is kinda why you need to check up on these things rather than making assumptions.

Every effect IO bonus only works for a limited time after the activation of a click power. If you want full time protection, slot it into a toggle. You not understanding how IOs work isn't a "bait and switch".

26 posts in and I'm still trying to figure out where the "bait and switch" comes in. Other than it being a hotbutton phrase to attract attention.

It's a problem that has been around since EQ, possibly before. If there were a way to get rid of them completely, it would have been done by now. Unfortunately, there are no ways to totally lock them out without inconveniencing a lot of innocent people in the process. "Throwing out the baby with the bathwater", so to speak.
