If you were playing any character with any sort of aggro aura, enemies you run past, even greys, would aggro onto you (they might be too lazy to follow if you go fast)... so if someone's ambush was headed in a direction and crossed paths with you, that'd explain that occurring.

It would appear that at the very least your video card's drivers are out of date, you should be able to get more recent ones from nvidia's website (might as well get the low hanging fruit out of the way).
Also, I recommend using CoH Helper and HiJack This over dxdiag, since dxdiag is obscenely verberse and the others show more information useful directly about CoX (plus, it highlights potential trouble areas in yellow)
Edit: You could also try running (from the Run dialog, accessible by pressing Win+R) the command C:\Program Files (x86)\City of Heroes\cityofheroes.exe" -project coh -console 1 which'll directly start the client with the debugging console enabled. As the game is loaded, it'll output to it whatever it's currently initializing, so when it freezes, you could write down what it gets stuck it, which at the very least could give us a hint at what's wrong. -
It seems like a good number of people are having issues with this over the last week or two. The problem isn't universal (I've yet to be able to reproduce it), and doesn't appear to be on the client side, either.
If you've already fully updated your City of Heroes installation (if you've played anytime in the last few weeks, at least, it'll be current), you can bypass the updater and launch the game directly.
To bypass the updater in 32bit Windows, you'd press Win+R on your keyboard (which brings up the Run dialog), and copy & paste in "C:\Program Files\City of Heroes\cityofheroes.exe" -project coh then press enter. If on 64bit Windows, you'd do the same but use the command "C:\Program Files (x86)\City of Heroes\cityofheroes.exe" -project coh (if you're unsure of which you're using, you can just try both... if it's the wrong one it'll say that the location is unavailable or an error along those lines).
If you're running OS X, you can bypass the Updater by open the Applications directory, going into Utilities and opening Terminal.app. In there, copy and paste the command /Applications/City\ of\ Heroes.app/Contents/MacOS/cider --use-dos-cwd c:/coh -- c:/coh/cityofheroes.exe -project coh and then press enter.
All three of those commands assume you've installed it into the default directory. If you installed it somewhere else, you'd need to modify the path to point to the correct directory. -
Basically, a 'min/maxed' build is an optimized one for maximum performance. A 'min/maxer' would be a person that makes (or tries to) min/maxed builds.

2-3 End Reds should be enough, although for especially long journeys (say, shadow shard), you might have to slow down your teleports a bit between jumps when you get closer to the end (not long enough to fall, but... closer).

I also did a full search for the files mentioned in the error and they do not even exist I believe they are created when you first log in fully along with the player_name directory I am stuck at the splash screen???? -

I'm not sure how defense and accuracy interact. Will have to look into that deeper.
So if they had normal tohit, and you were at 45%, they'd have a 5% chance to hit you before taking accuracy into account (50-45 = 5). If we say they're a generic +5 minion, then that puts them at a final chance to hit you of 7.5% (5 * 1.5 accuracy = 7.5% final chance). -

Yep, exactly. They're only really special in that virtually every attack is counted as one of the three, so generally they give more bang for your buck than damage types (unless you already have a bunch of damage-type defense).

I believe this is a known bug.
Also, if you're wanting to take a screenshot of the character creator/enhancement screen/costume editor/anywhere else that's not the in-game part, you have to press 'print screen' on your keyboard, then open an image editor like Paint or Photoshop, then Paste the clipboard contents into it, then you can save that. If doing that you get an all-black screen, you can using the Windows Snipping Tool (under Accessories in the start menu on Visa & 7) which generally works in those situations. -

I do agree that the updater has some glitches, but IMO it's still better than WoW's in my experience (with CoX's updater, I've always been able to just start it then forget about it for a good while... with WoW's, I have to click 'accept' and 'next' a few hundred thousand times to download every single minor and major patch since the game was created!). I wouldn't mind seeing a rewritten updater, and I've actually been tinkering with completely replacing it, but part of the problem seems to be with the updater server, as well as the upstream ISP. -

If it's in a folder named 'data', you just drop that in the City of Heroes directory, otherwise you make a folder named 'data' and drop it in that (unless it has an installer, in which case run that).
It doesn't have to actually replace the files, since CoH searches in the data/ directory then in the .pigg files for what it's looking for. -
The binds I use are (taken from a bind file):

You have to double tap the key, but I find it simpler than pressing two keys (or three, if you also have ID... or 5, if you're a thermal) -

Okay... so what's the issue? Has the download slowed down to an incredibly low amount? If it's been like that for a while, you could just close the Updater and restart it, and it should speed up.
