While I could see Koh being behind it all, it also seems just a bit too obvious. (Mask = Koh)

I would assume his relation to Avatar Aang is what makes his resistance to bloodbending possible.

So, to take an accounting of the original cast, we now know that...
Aang is gone (duh!)
Katara is still alive (about 84 years old now)
Sokka is gone (as stated by Katara)
Toph is a little unclear. Her daughter Lin is head of the RCPD, but Toph's own status is yet to be confirmed. She could still be alive at 82.
Suki? Unconfirmed.
Zuko? Unconfirmed, though we know his VO has a role in this series that is not Zuko, so it doesn't look good.
Mai and Ty Lee? Unconfirmed, though it has been said that the Equalists will use Ty Lee's chi-blocking martial arts against the benders.
Azula? Unconfirmed.
