"Star-ing" a player?
You can right-click on another player and select "Add Note"; it opens up a window that lets you add a one-to-five star rating and a description.
It attaches to the player, not the character, so in the future, you can see what you thought of the person behind the keyboard, and it's all stored locally, so it's just for your own personal reference (nobody else can see it).
In the options menu, you can even set it to show a person's star rating all the time. I like to turn that on so I can always see who the five-star and one-star people are.
FUN FACT: That burst of light when you level up is actually the effectiveness escaping from your enhancements all at once.
If you click on a player's name (or on them I think) there should be an option to add notes. Notes are viewable only by you, so it's a way to tell yourself whether you liked playing with this character or not. You can rank them 1-5 stars, and then if you go into options you can set it so that you view a player's rating always, or when you mouse over them, or whatever. I like to set that to always so I don't miss that I've rated someone or not -- usually it's just a way for me to recognize when my friends are on an unfamiliar alt.

I keep hearing about "I Just two starred this player" or something like that. What is that? How do you do it? What does it mean?

It gives you a chance to make a notation about the character, for your use. You have the option to give the player a rating, 1-5 stars (or none if you don't use that feature.)
In your Options menu, you can set the interface to always display the number of stars you've given a player, or only on mouseover, or never.
So, in a good case, you team with a person, add a note and give them a 5 star rating. If they are on with a different character, you might (depending on how you have your setup) see this unfamiliar character with a 5 star rating. You know its someone you rated highly in the past.
I like using the feature a lot.

Right. You can left click on a player's name in chat (any chat, afaik), or you can right click on the player themselves if their character is on your screen. I usually find chat more useful, and easier.
You can also left click on a person's name in chat to invite them to your team, send them a private message, etc. It's one of the more handy features that have been added to this game.

Something to realize:
If you use more than one computer, the notes don't automatically follow. In your COH directory, there's a playernotes.txt file. It's just text, but you'll see the players, notes and ratings in it. Copy it somewhere if you want to keep that if you need to reinstall or something.
Each line (if you look at it) is a new player. It goes:
"Global" "Rating" "Other names" "Notes (or (Null) if there aren't any)" (though it tends, to put, a comma, where, spaces, go, in, other, names.)
So, if:
1. I team with @Newbie McNewberson on his Newguy character, he's not exceptional but I want to note I've played with him before and keep an eye out for him - 3 stars. I team with (or get info) on him two more times when he's on Nooblaster and Nootank.
2. I then run a TF with @Egotank who has zero clue what he's doing, is generally irritating, and I don't want to team with him again (1 star,)
3. but someone else @Brootalic on their Slicylicious character takes over and is just awesome, so I five star them, my playernotes.txt file would look like:
(All names made up.)
"Newbie McNewberson" "3" "Newguy, Nooblaster, Nootank" "New guy, not bad, talk to SG about inviting."
"Egotank" "1" "EgoTank" "Ugh. Buy stock in tylenol before ever considering teaming with him again. Awful, bossy, won't listen."

Wanted: Origin centric story arcs.
If you've only played an AT once (one set combo) and "hate" it - don't give up. Roll a different combo. It may just be those sets not clicking for you.
Additional info. If they change their global handle, the player notes and stars will not change. They'll still be attached to the old name.
I recently had one person on ignore and I couldn't figure out why until I did a grep on my logs on the character name. That's when the original global name popped up.

I give everyone I team with a rating. My system for rating other players is both for recognizing friends and SG mates in-game as well as rating the behavior of players I team with in general:
5 stars: Supergroup officers, and people I know from outside the game (either in real life, or from non-CoH related online hangouts).
4 stars: Supergroup members and other regular teammates, and players I've teamed with who have impressed me in some way.
3 Stars: Default rating. Everyone I've ever teamed with starts with 3 stars, and could be upgraded to 4 stars if it's someone I end up teaming with regularly, or downgraded to 2 stars or even 1 star if they make a nuisance of themselves while I'm on a team with them. This is the only rating I don't create an explanatory note for.
2 Stars: Idiots. A 2-star player is someone who I didn't enjoy teaming with, but I'm still willing to give the benefit of a doubt and team with in the future, hoping the objectionable thing they did was an isolated incident or that they've seen the error of their ways since the last time we met. But I wouldn't join a team led by one.
1 Star: Irredeemable. These are the lowest of the low, who are either consistently stupid or outright malicious. This rating goes to griefers, trolls, RMT spammers, and for harrassment and offensiveness in general. I usually follow up a 1-star rating with a petition to the GM's about what they did to earn that rating. If I give someone a 1-star rating, I will never team with that person again.

You can also use /playernote to write notes for yourself. If the 'player name' is not a valid name, a note is still created. So, you can create notes for yourself.
E.g, I use /playernote timer to keep track of when my toons did the SSA to keep track of the 7 day cooldown on the Alignment Merit reward. Just be aware that when you call up a note and it's blank... scroll up - the info is still there.
