The original difficulty slider levels were heroic, tenacious, rugged, unyielding, and invincible. They very roughly corresponded to +0x1, +0x2, +1x1, +1x2, +2x1. However, *before* the slider came out difficulty scaled with team size. If I remember correctly, the critters scaled upward +1 with 4, +2 with 6, +3 with 8. When the slider came out, it *stacked* with this intrinsic difficulty and so if the leader was set to invincible and you were in a team of eight, you'd be facing +5s.

I myself tend to get the IO Procs at the lowest level possible, and usually as soon as I can (Steadfast Res/+3%, Miracle and Numina Procs, ect) and set IOs at their highest (Kinetic Combat at 35, for example). Until then I rely on SOs/Common IOs, and they often make a huge difference in what I can do.
If I'm playing a Firey Aura or Dark Armour build then I will get -KB IOs ASAP, even if I need to e-mail them from another toon.
But I would agree that, at standard difficulty, the game is balanced around SOs, or at least with SOs in mind. The higher difficulty sliders are there for those who want a challenge or with speced out IO builds.

i'm pretty sure she was referring to the rule specific to the spelling of Arcanaville, not the more general grammatical rule. The fun thing about most grammatical rules are the exceptions to those rules and the rules that apply to the exceptions.
