I hate to be the one that says it, but it seems like a Forum Game. Take an established NPC, stick his/her name in a blender, and make up a reason for someone with the new name.

I'm surprised nobody's mentioned the mysterious NPC in Nerva. She seems almost like the Woodsman, but not quite - the Woods Wight, always bemoaning the loss of the tower-like tree in Mercy she used to be bound to.
Then there's the former friend of Hero One, now piled under costume requests - Laden Jay.

No, really, I mean it.
OK, so we now know that Honoree is really Hero One and we all strongly suspect that Mender Silos is really Lord Nemesis. Have you noticed any other characters in the game who took their name, threw it into a bag of Scrabble tiles to come up with a new name?
How about in the ITF, the lieutenant named Roman Tice who suddenly appears, shoots the team leader with an arrow and then just as quickly disappears, never to be seen again?
Or how about the Flight 815 story arc, when you finish fighting the Lost, you find a very familiar looking faceplate/helmet in the treasure chest of St Namaste.
But the one I really like is the new APP/PPP power (available to trollers and MM's): Call Baby Krawler. The summoned toddler doesn't really resemble BaB's all that much (except for the cute lil bionic arms) but the fact that the animations work perfectly is a dead giveaway.
That's all I've found so far; how about you?
