Auto-fire green circle - not working consistently?
Keep in mind that the z key by default will unqueue a power, and will remove the green circle, so if you're hitting z a lot by accident that could be the problem. Autofire also can't be used on toggles or powers that need to be placed with a target ring.
It's a known issue. It will usually start firing off once you go back into combat mode.

As someone posted, zoning can affect it.
Also, some or all powers will auto-fire oddly based on your targeting. I've only noticed it on my shield tank as I don't normally set stuff to auto-fire. If you are targeting nothing or an enemy, it will work correctly. If you're targeting a contact or object, the auto-firing power simply will not trigger - until you change your target.
Fix coming? Who knows.

I've found that powers will not autofire if a non-combat NPC (such as a Wentworth's employee) or object (like a mish glowie) is targeted. Once I hit Esc to clear the target, the power fires.

This, sometimes mine wont go off at all for any reason, but the little green circle will still be there. I have to click it again for it to go off. Been like this for a long time now.

I actually kind of like this bug, it's cut down on heals spams at WW significantly.

It's been a bug since i14 Beta. They're not sure why it's doing this and since it's not critical for most characters I think it's not at the top of the bug-priority list to figure it out.
A fix will come eventually, but in the meantime you can also untarget (hit Esc) to get it going again.

Similar experience.
I often log off with Practiced Brawler on autofire.
Sometimes I log on with PB still on autofire and it would not fire until I either manually click it or I hit my key bound to "/powexecauto Practiced Brawler" to turn off, then on the autofire.
But this doesn't always happen.
Not a big deal to me, just odd.
