If i had 2 different powers, say Haymaker and Knockout blow, and slotted them both with 4 mako's. Would i get the bonuses from both? Would it equal +1.5% HP/+3% damage or +3% HP/+6% damage?

Yes you would get the bonuses from both and they add.

You can stack 5 of the same name bonuses from the same or different sets

Example the 5% recharge from say red fortune, doctored wounds and crushing impact is 5 combined, which gives you a maximum of 25% in that case.

as long as they don't have the word "unique" in it they add from both powers. Unique only one time on one toon.

Does the magnitude of the bonus still count towards the limit of 5? For instance, could I have 5 of X% recharge and another 5 of Y% recharge?

You should get bonuses from both.

You can use the attribute monitor to make sure.

You can add the same "little bonus" up to five times. No more than five, even if they're from different sets. 3% Resistance and 3.11% Resistance are different little bonuses!

What defines "small, moderate, large, huge, ultimate," etc. for the various bonuses? I'm assuming you mean that you can have 5 "small" of the same bonus, as well as 5 "moderate" of that same bonus type. I'm just wondering if there's a guide that explains these differences in real numbers.

The naming comes from the Combat Attributes window.

The Rule of five is as thus :

Castle further elaborated on the rule of five [Originally thought to limit a set bonus to five of the same value] in I13 Closed Beta saying that the exclusion of more than five of the same bonuses is based on the name of the bonus and not its value. For example, a character can benefit from up to five global benefits of Luck of the Gambler: Defense/Increased Recharge Speed, which gives a global +7.5% bonus to Recharge Rate, as well as up to five set bonuses of +7.5% Recharge. That's because under the Real Numbers in a character's Combat Numbers window, you will see the Luck of the Gambler with a total of 37.5% Recharge bonus (5 x 7.5%) and Huge Recharge Bonus with 37.5% Recharge bonus -- two different named bonuses, even though they have the same value bonus for the same attribute.

That quote pulled from the wiki page Catwhoorg linked to.
