What video card and driver version are you running with?

I get the following errors on startup after patcher has finished and i launch game:

I have the latest nvidia drivers: 197.45 and thats what nvidia recommends for my setup. Shortly after those pop up a black small coh window appears and either hangs with a crash report or just hangs in general. -
The safe-mode check-box is pretty bare bones. My guess is that it is a driver issue. But, I see that ATI Mobility Radeon 9000 drivers are hard to find. You may want to try the -useTexEnvCombine as described here:

You should only use it if you can't get into the game (game crashes at startup) and your drivers are already up-to-date and you are using an Intel integrated graphics chip. -
I17 includes a reworking/updating/tweaking of the graphics engine. Did you try my suggestions above to disable any overrides in the ATI Catalyst Control Center?

Change the settings in the ATI Catalyst Control Center to allow the game to control the Anti Aliasing. There may be other settings which should be set to "Let the application decide".

Make sure your video card drivers are up-to-date for starters. If that doesn't help, let us know what kind of video card you are using and the driver version.

Also make sure you are using recent drivers (preferably the latest version, but at least from sometime this year). There have been numerous problems/crashes which have been tracked back to old drivers. Newer drivers may also improve performance.

Note that this is mainly for people with integrated Intel video and unable to get into the game after making sure their drivers are up-to-date. This is not for people who are able to get in the game already.
Also, a mention of safe mode might be handy:
"When clicking on I agree from the second launcher window, there's a little box that says Safe Mode." -
If "Ambient Occlusion: Off", then the rest of the values associated with Ambient Occlusion (Strength, Blur, Resolution) are ignored and Ambient Occlusion is really turned off.

I would recommend not using the NVIDIA control panel to override the antialiasing and possibly other settings. I don't know if you've noticed, but the FSAA option was moved out of the Advanced Graphics Settings.
So, try setting all of the options in the NVIDIA to "Application controlled" or whatever it's called and run the game. Keep in mind that we did not test the game with any overrides from the NVIDIA control panel or the ATI Catalyst Control Center. If the game runs fine without any overrides, then the problem is with the way the override is implemented which is out of our hands. -

If you push "Cancel Changes", it will not save your changes. To close the options window and save your changes, click the red X in the upper or lower right corner.

Also, shadow maps are very CPU intensive. Having a faster CPU (and/or more memory) may help improve the frame rate when shadow maps are enabled.

Kioshi, have you tried the test server recently?

Have you ever noticed that when you are standing in the shadow of a tree and can't see the tree, you can still see it's shadow?
Or, you can see the shadows of objects that are around the corner of a building, but you can't see the object casting the shadow?
Would you rather these shadows pop in/out as these objects become visible/become obscured or out of view?
Also, determining if an object is obscured is not cheap/easy.
In practice, it does sound easy. In reality when you try to implement it, it is not nearly that straightforward and not that easy to get working at a decent frame-rate.
If the performance if shadow maps is unacceptable, you can always fall back to the stencil shadows (this is the legacy shadow mode from pre-I17). -
EDIT: I didn't read your reply completely. Maybe try running some OpenGL tests?

Try uninstalling your video card drivers, running a driver cleaner tool, and re-installing your drivers.
The other games you've mentioned use DirectX and City of Heroes uses OpenGL. The symptons you've described and shown may be due to a problem with your OpenGL drivers which are a part of the video card drivers.
