This indicates a hardware issue, likely due to excessive heat.
Change the Nvidia graphics card out if you can and see if the problem persists.
I'm tempted to roll back the drivers - just have to dig out the older install files.
EDIT: Last update I'd run was 266.58. I'm presently rolling back to 260.99, which, IIRC, was my last stable build. -

No, there aren't any FX that are triggered for damage.
Some procs (chance for hold, chance for resist debuff) do have visible signs that they're working, though. -
Note, however, that for "Chance for X", where the chance is less than 100%, click powers that can be activated at least every 10 seconds are the BEST choice - the faster, the better (eg: Chance for [Type] Damage, Chance for Recharge, Chance for Build Up)

Health, hands down.
The Numina's will always be providing the bonus in Health. You'll only get it for 120s after activating One With the Shield (leaving at least 240s of down time). -
Accuracy is theoretically useful until you reach +1800% (At which point you have a 95% chance to hit something no matter how high its defense is).
You can't have that much accuracy, however, and ToHit is generally more useful (unless your target has more than +45% defense, in which case ToHit has no effect against the "extra defense" of your target).
Those are the most extreme cases, however, and they don't account for PvP Elusivity.
If you want a 95% chance to hit +4s without any built in defense or ToHit debuffs, you will be looking for around 143.5% accuracy (not accounting for ToHit buffs and Defense Debuffs). -

Yes, because it's easy to strip a Tanker of aggro when he's got (13.5 * 1,000) more threat than you than (45 * 1,000) more threat than you.
Let's use some numbers here. A Blaster deals 200 damage with an attack with no debuffs, and a Tanker does 100 damage.
Blaster Threat: 200 * 1 (AT Mod)* 1 (DebuffMod)* RangeMod * AI Mod * 1 (TauntDurationRemaining * 1,000) = 200 Threat
Tanker Threat: 100 * 4 * 1 * RangeMod * AIMod * (13.5 * 1,000) = 5,400,000 Threat.
You're telling me that the blaster would pull aggro from him? IF you can show me that in-game, then it shows that something in the formula is broken, not that the Tanker needs Taunt.
Taunt just gives the Tanker a higher modifier. It is not an instant and permanent aggro-gaining tool.
Fantastic. Even if you hit completely different targets with gauntlet and Taunt, you're only up to about 15 targets. If the Blaster opens up with an AoE and hits 16 targets, that's still one guy who could potentially turn and shoot the Blaster. And heck, if you do hit the same exact targets (especially since your first attack and Taunt are both hitting at least one person in common), then there are potentially six - seven enemies that could turn and hit the Blaster.
Taunt isn't an "I win aggro" button. It helps, for sure, but you've still got to be smart about aggro. -

While the Taunt (and Confront, and similar) powers wouldn't be bothered significantly, a taunt aura could fade out nearly in its entirety as its potency is based on duration remaining. Especially Willpower. -

Assuming you did mean GB, rather than MB (yes, computer systems have advanced their memory capacities by 3 orders of magnitude over the last 15-or-so years), 4GB will work quite comfortably for a 64-bit system that's running a single instance of City of Heroes and not much else. If you want to dual (or triple, or quad, etc), or if you want to run other applications in the background, the extra RAM will help.
I run the game with a 512MB dedicated video card (9700M GTS), and can manage on low Ultra Mode settings at about 30 FPS, assuming that I'm not running much in the background. I doubt that memory capacity is my bottleneck, but most of the more modern video cards tend to come equipped with 1GB dedicated memory, and would probably handle Ultra Mode better. Resolution can be a big determinant of your graphic card demands. My laptop runs at 1440 x 900, and I experience significant loss of performance when I hook up to a 1080P HD TV (1920x1080 resolution)
Processor speed is my current system bottleneck, at least according to the Windows Performance Scores - I'm running a 2GHz Core 2 Duo, which is rated a 5.5 on Windows 7's scale (all of my other components rate 5.9). For purposes of City of Heroes, you'll generally find that the sum of up to two processor speeds gives you a rough indication of how much processor power you're throwing at the game (though other features, such as cache memory, 4 cores instead of 2, etc, will probably carry some benefit). Reading up on the performance of the various processor lines may prove enlightening as well - there's more to a good, fast processor than just a high clock speed (though high clock speed helps, a lot)
For reference, the laptop I use is a Toshiba Qosmio X300-01u, upgraded to Windows 7 and with a second 640GB Hard Drive added in the secondary Bay (and separated from the other hard drive with a couple of Pog slammers I have from way back when...they're handy for that, provided I don't shake the thing around too much) They've since released the X500 line, which packs a lot more processor and GPU power, and displays in much higher resolution with another inch of screen real estate. If my income wasn't being used for more important things, I'd probably be splurging on one of those, or one of the similar ASUS gaming laptops (depending on pricing differences). -
Also, group AI has been set up to split up among your entire team once the group has been aggroed. All it takes is a scrapper throwing the first punch, and it's like you're not even invisible anymore - some of them WILL go after you. (That said, if someone else catches them with a taunt effect, they will most likely stop paying attention to you and start pounding on the taunter)

In that case, I'd suggest a Petition and Bug Report. Something might be flagged wrong on your character.
