Can I buy old cheap starter boxes for the months?

I'm pretty sure you can only apply a code for each update/release to your account once.

I tried it once, even made sure to buy a version of the game that I have Not applied - Still didn't work. It does work if the game version applies some Unique extra effect, like Collectors Edition, GvE, or GR. However, you can only apply those once.

City of Heroes ( including COV ) can now be bought for a buck on Amazon.
It makes a great Premium account gift to friends that want to play "City of Heroes: Freedom" for free each month in the near future, and will allow them to have more than the two character slots totally free accounts will have ...
( I think?)

Just make sure the box has not been opened. Because if the box has been opened, then it's likely that the code has been used. Descriptions like "played once" tip you off that the code is no good.
What about the european and italian collector's edition that are for sale on the official store?
Can we get the extra costume pieces and month of game time for those even if we're in America?

Well, sort of. I mistakenly ordered a European COV box on Ebay. I could use its free month, sure, but it's a free month on a Euro account, NOT my usual American account.

If you already have that edition unlocked you don't get the extra month. It doesn't matter what language it's in, or if its NA or EU. You can't double up free time on the same edition.
For example if you already have Good vs Evil unlocked you won't get any extra time if you buy the UK version. They are still the same edition.
Now if the edition comes with unique perks like a costume piece or temp power that isn't unlocked yet on your account, then those will be unlocked.
For example if you can get your hands on a copy of the Architect edition you can unlock either the Magic or Cyborg Booster for free. As long as you have't bought them already. If you already unlocked them both you get nothing.
A number of starter boxes of various editions of CoH are available online rather cheaply (well under $15). Even though I already bought and applied them the first time around, would I be able to buy up some of these old editions (like Good vs. Evil, Architect, etc.) and apply their codes to get the benefit of that month of play time?
