Her immunity to the Ghost Slaying Axe is much like Stateman's or Mako's crashless Tier 9's. It's cheating, specifically designed to make the encounter challenging, whether you agree with the principle or not.

Ghost Widow is connected to the entire Arachnos organization, so she cannot be disrupted nearly as easily as other spirits, as her tie to reality is not nearly so tenuous as most ghosts.

Mind you, if you could go back in time to the day she died the axe might have a bigger effect on her. Of course, you'd also have to watch out for Wretch.
There are also hints that she is, in some way, "special". When she tries to return to life in her Patron arc, she says that she ends up with an "ectoplasmic body". This may be why she appears solid, and not ghostly like War Witch. (And I'm pretty sure any version of War Witch that can be attacked by the Ghost Slaying Axe wouldn't be effected by it, either. There are perks to having the lead developer as your player.

They forgot to make her count as a ghost for the purposes of the axe. It's really that simple.

I'm just curious about this: Why is Ghost Widow not affected by the undead slaying temp (and vet) powers we get?
Is it a story reason? Like because there's something special about her as a ghost or maybe she's not as undead she thinks, etc. etc.
Or is it a gameplay reason like too bothersome to make multiple ghost widows who are and are not affected, to prevent a gangup of GSA vets in RV, she's a 'special AV' and that means being an exception to weaknesses of standard character, etc. etc.
