Veteran rewards (1/year of paid time) and purchase options. It's on the page after you select the server, but before you pick your character.
You use "Character Slot" tokens. Everyone got 2 free ones with I12 (this should apply to newer accounts as well), you get 1 free one every year through veteran rewards and finally you can purchase them for $4-$6 each depending on how many you purchase at once.

Anyway, to back up Adeon, I can say that it does have 2 redeemed slots on my main server, and because I never really kept it active, it has not even reached it's 1st year Veteran yet, meaning.. it was given the initial 2 slots from i12, which was released back on May 20th, 2008.
Whether or not it still applies to new accounts today, I cannot be sure. i don't have any other more recent accounts to verify with.
The two free slots are granted via a script - they've never mentioned turning it off.
