Okay, the first thing I see. Blazing Aura. It's just not slotted. No damage enhancement and no accuracy enhancement.
As such, you're missing. A LOT. Even though you have endurance reduction slotted, you're wasting endurance with all the missing that's happening.
While the damage per-hit on BA is tiny, over time, when properly slotted, you'll notice that it's one of your largest damage-doers. You get this AOE early for a reason. You may finish someone off with the axe, but BA is what really will have killed them (or with high-regen enemies, prevented them from healing damage back).
Also, pick one mode of defense (typed or positional). The way you're slotted makes it look like you're trying for both. You don't need that and it's causing you to waste slots.
Additionally, you've ignored Fiery Embrace, another damage booster (and one with twice the duration of Build Up).
What I'd do is stop trying to build some of your attacks towards recharge time. Convert the triple LOTGs to the global recharges, and reclaim some slots for other things (like slotting BA).
Additionally, changing out all the Scirroco's acc/dmg for dmg/end will help immensely. You're biggest end use is in attacks. For defense IO's I like the GotA's as you can get +rec and if you 4 slot it +end. Not a fan of Manuever's for tanks, not worth the return vs. end cost (IMO). Prefer to 4 slot Reactive Armor in the resist powers for the s/l bonus. Multistrike works well in Fiery Aura. Don't have time for much more of a break down, but that's my quick 2 cents worth.

I think your number one priority should be increasing your S/L defense. You have several Aegis sets slotted for F/C defense, but since your fire resists are capped anyway, the defense is mostly helping against Cold damage--a very rare damage type.
The first thing I'd do is four-slot Fire Shield, Plasma Shield, and Tough with Reactive Armor. (3 slots won't give you the S/L def bonus, and will probably leave your shields underslotted.)
Slot as many sets of Kinetic Combat as you can afford. Personally, I think the cost/benefit ratio of Maneuvers is way too low; you'd probably do just as well picking up another ST attack and slotting it with a KC set.
If you can find three more slots for Taunt, 4 slots of Mocking Beratement has a 2.5% S/L bonus.
By the way, I usually slot my damage auras as an attack, but heavy on the end reduction. If you don't have the slots to do so with Blazing Aura, it should at least get another End Red slot.
Are you a big fan of knockback? IMO, KB IOs should be the last choice in additional slots, since they can turn Axe's KD to KB. If you're crunched for slots, (and you are) a single Acc/Dam or Acc/Dam/End from another melee set will round out the KC slotting.
With the changes I mentioned, you'd go from 25% S/L def to 31%, not a huge change, but every percentage point closer to 45% becomes increasingly more valuable. How close to the soft-cap you get depends largely on what you're willing to give up to get to it; for instance, 4-slotting Kick with KC would help, but you would have to steal slots that you're using elsewhere.
Hopefully someone with more FA expertise than I have will chime in, but I do think your best bet to increase survivability is to get your S/L def as close to the soft-cap as possible.
EDIT: forgot to mention, another place to free up slots is to five-slot your PBAoE attacks. Your set selection is fine, but I'd suggest Dam, Dam/Rchg, and Acc/Dam/Rchg from Eradication and Dam/End and Acc/Dam/End from Scirocco's.

It looks better in some ways, but not so much in others.
Just looking at your endurance costs of these three attacks: Whirling Axe, Cleave and Pendulum. Right now using them ONE round costs you 34.4 endurance.... Yikes. You seriously need to change these. Oblit's are great for damage and for the s/l bonus, but are horrible on end. redux. Changing them to Multistrike's alone changes your end cost for a single round of attacks to 23.01 end giving your recovery at least a chance to catch up. That's a savings of over 11 pts of end. per round of attacks just in those three and you don't loose a lot on your s/l defense.
If you can find a way to get a Numina's +rec/+regen Unique, grab it along with a heal and put them into PP. Get a Miracle +rec and heal and stuff them in your health. Without the addition of the Atlas Medallion and Portal Jockey Accolade those will push your recovery to 3.29 end/sec. The addition of those two accolades will send it to 3.62 end/sec recovery. This is compared to the 2.78 end/sec you're currently getting (not including what you'll get from the P.Shift proc).
Adding in the Freedom Phalanx and Task Force Commander accolades will push your total health up by 370 ish points and shift your regeneration from 21.8 hitpoints/ps to 25.8 hitpoints/ps. It's not huge, but every little bit adds up.
I'd take two slots off of combat jumping and put one into build up and use Guassian's 6 slotted. (nice defense bonus!)I'd take the second slot and throw it into Weave for more defense. Again, I like taking Gift of the Ancients for the rec and +end bonuses, but YMMV. All of this pushes you're s/l defenses to over 31% and gives you more then a + 2 end/sec recovery.

Plinked around, made a build, read Rangle's comments, and then made adjustments where necessary (the Oblits). I did keep one set of 5 in BA just for the bonuses though. The order of powers/slotting can be shifted around - I was more interested in the end product.
Added a few powers and dropped a few. No purps. Let me know what you think, I'm trying to improve my Mids-fu.

Have you thought of putting a force feedback chance for plus recharge proc in some of your AOE knockdown attacks?
Would get healing flames back quicker.
Multistrike is a much better set for damage auras than obliteration.
Obliteration is for nukes - it maximizes recharge over end redux (makes it good for lightning rod and shield charge)
Blazing Aura is the absolute *worst* place to put a set of Obliteration. It has almost no end reduction, and in a high-end toggle like BA, you should be aiming for at least 50-60% end reduction.

I completely agree with Rangle that although the Oblits have a useful bonus, the lack of end reduction is a big problem. It's a great set in certain situations, but since you can get the same bonus with 4 slots of (much cheaper) Smashing Haymaker, I don't think it's a good choice for typed defense. But rather than Multi Strike, I'd recommend Eradication for the E/NE defense. It has the same low end reduction problem as Oblit, but it's easy to remedy since you only need three slots for the bonus.
Here's what I would do, but keep in mind Rangle's advice about the recovery uniques, accolades, etc. I also prefer GotA to LotG, but that's more personal preference. I did like the OPs slotting of Blazing Aura, so I kept that. (It's pretty difficult to reach that level of end red with Eradication.)
I also dropped Maneuvers to pick up Gash and re-arranged things a bit. The KCs in Gash more than make up for the lost defense in Maneuvers, and I think another attack is much more useful. This version has 36.5% S/L vs. 32.8%, and will be much easier on the end bar.

Well, the power choices vary from the OP's, but this run's at nearly 40% S/L defense and has 122% Max HP. Get your accolades, and that can be 142%. Those two things plus resists will make you pretty darn tough to kill. And with a heal on top of that, you should be untouchable in most every situation.
Oh, and enhancement values on your powers have not been sacrificed.

I've never been able to convince myself that spending 6 slots for Multi Strike on a typed def build is worth it for a .94% F/C and a .94% S/L def bonus. Two sets (12 slots!) will get you 1.88% def of those four types.
Rylas, if Whirling Axe and Pendulum were slotted with 3 Erad and two other set IOs, you'd get 6.26% E/NE def instead. Throw the two unused slots into Build Up and slot 2 Rectified Reticle and you'll get...a 1.88% S/L defense bonus.
Same number of slots, either 1.88% S/L/F/C defense *or* 1.88 S/L and 6.26 E/NE defense. Hmmm...

On top of that if Rylas takes the slot the Steadfast -kb is in on the Plasma shield, moves said slot to Fire Shield and switches the -kb for the Steadfast unique in Temp Protection he gets an additional recovery boost for the same IO's and slots. (Ending up with the pairing of one Steadfast -kb and the def/resist unique in Fire Shield and the second Steadfast -kb in Temp Protection.)

Finduilas, I'll do you one better and make it a 4 slotting of erads for more HP as well. The left over extra slot is now in Stamina for a little more recovery (about .1 end/sec). The OP can slot WA and Pendulum as I've done Cleave, or go for the end discount. It's all on preference at that point.
Rangle, the OP can certainly go that route if they like. In my opinion, .03 end/sec won't be noticed, which is why I didn't bother. But there's certainly no harm in it.

My usual slotting for PBAoEs is Dam, Dam/Rchg, Acc/Dam/Rchg from Erad and Dam/End and Acc/Dam/End from Scirocco's, which fully slots the attack and gets you a 10% regen bonus from the Scirocco's.
Your way works too--the additional HP is nice--but it's a little light on damage enhancement for my taste.

Since Firey Aura has no +def of its own and Weave and CJ are already taken, might it not be better to drop the attempt to slot up the various Typed defenses and instead slot for Positional? With only 3 aspects to slot for instead of 7 it's a lot easier to reach high values in all of them, and you don't need to wrangle slots around trying to eke out a few extra % here and there.
For example: FA/BA has several PBAoEs, so you can get 18.75% +Def (Melee) by slotting 5 sets of Obliteration and will be covered against ALL Melee-class attacks rather than just the S/L ones. While it's true that many melee attacks have a S/L component not all of them do, so when you run into ones that aren't you're still covered. Since some of those same attacks belong to AVs, when they hit they HURT so having defense against them is a good idea.
Since almost all attacks (about the only ones that don't are a few from Mind Control) have a positional component, by slotting accordingly you're also providing protection against Psi attacks without needing to sacrifice half the build or more to get good values against it.
I realize that my slotting example for Melee def is a bit low on End Reduction, however it seems the OP will be slotting the Numina and Miracle Uniques as well as a PS Proc, and I know from experience that that will be sufficient to cover any recovery shortfall.
Most of the time with typed defense you're mainly focusing on S/L but trying to add in Nrg/Dark and some F/C as well. Most of the attacks out there have a S/L component so not only are you covering a great portion of the melee attacks, but also ranged and AoE attacks that have S/L in them. I don't recall what the percentage of attacks that have those, but I believe it's in the 70% range. (don't quote me on that)
So if you get to the same percentage with S/L defense compared to just Melee defense you have greater coverage against more attacks.

Except that I've found that it's not nearly enough to do that, but then I assume I will be facing foes using non-S/Ls attacks frequently since a fair amount of upper-level content is against Rikti/CoTs/Carnies. Now if you're talking the ITF then S/L is clearly the better choice since that's all they do.
I also assume I will be tanking AVs, and a fair number of those use non-S/L attacks. Gratned that's a fairly small percentage of overall content, but for 40+ it's a much larger percentage so I feel it's worth taking into consideration given that the endgame is usually what such slotting is aimed at.
Then again I believe in slotting to cover as many aspects as I can, so it seems odd to me to only try to cover 2 of 7 Types while going for Positional you can cover all 7 (more or less). I'm sure it will be sufficient for almost all cases, but then I build my Tanks for extreme survivability so I need the additional coverage while most other folks probably don't bother as it's a pain getting everything set within such narrow tolerances.

It's also noteworthy that typed defense bonuses are usually available for 3-4 slotting sets, instead of the 5 or 6 slots needed for a positional bonus. That leaves you with a lot more flexibility in slotting since you're not locked into 6-slotting everything for the bonuses you need.

On top of what Finduilas has said, (if the paragonwiki site is correct) most of the melee attacks of the three groups you've mentioned: Rikti, CoT and Carnies have either a Smashing or Lethal component to them, as well as some of their Ranged and AoE attacks. So focusing on Melee only covers a portion of their attacks unlike focusing on S/L which will also help with the Ranged and AoE attacks where applicable. S/L covers more of their attacks then just pure Melee. If I was working on a Shield Defense or Super Reflex toon, by all means I'd focus on positional. Starting with little positional defense, it's harder to get enough set IO bonuses to cover all three positions and make that effort worthwhile compared to focusing on S/L first, then adding in Nrg/Drk where possible.

I'd personally go on Nrg/Neg defense (35ish% should be ok paired with your resistances) since 70ish% s/l res should do fine with just any decent s/l def lvl (25ish%). That'd leave you with the cold and psi gaps, but when building up for def you raise both typed and positionals so I'd rely on whatever positional you get from raising other typed defenses for said cold/psi gaps, plus Battle Axe has decent KD/KU dmg mitigation (plus Burn would also help on that). With such a good self heal that recharges in no time, I personally wouldn't look for +regen, it won't hurt having it but wouldn't look for it.
If I was working on a Shield Defense or Super Reflex toon, by all means I'd focus on positional. ...please help me clean it up...please!
For concept reasons I created a Fiery Aura / Battleaxe tanker, I know, I know, but please stick with me. I've somehow managed to level him up to level 40 and I now need to try and IO him up as he desperately needs a boost and a good beefing up. I've got some resources, so short of purples I have the means to get some good IO sets. On hand I already have three Aegis sets for my resists, a couple -kb enhancements to plug those holes, a steadfast unique, kismet unique and regenerative tissues +regen. I've tried a couple different builds and respecced many times, but I just can't seem to make him stronger, tougher and able to dish out some damage. To be blunt, I've made a real mess of him. I know that the combo is rare, but I'm hoping someone with any experience in fire tanking can give me some help. And my last go round with Battleaxe was a nightmare as it sucked down more endurance than I could generate, and consume didn't help near enough.
At any rate, here's my last build to start with, I'm open for any changes in power selection and IO slotting.
