Mechanics Question
The way I understand it:
ToHit affects your player as a whole. Accuracy is part of the power itself. Both are used when deciding if your attack "hits" it's target.
If you increase the Accuracy of a power, then that power has a higher chance of "hitting", whereas if you increase your ToHit, then ALL* your powers will have a greater chance of "hitting".
When a power says it gives the enemy -ACC, I believe it's actually reducing the foe's ToHit... much like when CoT hits us with [Gloom]'s -ACC, and the Combat attributes shows a ToHit decrease, which affects the overall accuracy of ALL* our powers.
*except for powers that "ignore buffs"
Also the counter to "to hit" is defense.

If you're talking about IO set bonuses, those can increase Accuracy globally. Basically, the formula looks like:
HitChance = Clamp( AccMods × Clamp( BaseHitChance + ToHitMods – DefMods ) )
Where 'Clamp' keeps it above 5% and below 95%. For most powers, having around 60% accuracy enhancement slotted in them is generally enough for most enemies. With some ToHit buffs on your character, you'll need less accuracy enhancements.

The main difference is how they affect your character.
To Hit is the value compared/subtracted by your target's defense (and then set to min. 5% and a max of 95%). The resulting number is multiplied by the power's accuracy mod - so +To-Hit is always more effective than +ACC if you have a choice.

There is a complex set of mechanics behind "To Hit" and "Accuracy."
To oversimplify only a little, as my high school physics teacher used to say, you have (Base To Hit + To Hit Bonus - Defense) * (1.0 +Accuracy) .
Base To Hit starts at 75% and goes down against higher level enemies.
Against an even-con enemy, with no Defense, 33% Accuracy ("one standard SO") makes the formula 75% * 1.33, or just about 100%. You always miss 5% of the time, you always hit 5% of the time. (Like I said, I oversimplified...)
I think against a +3 enemy your Base To Hit is about 56%. Don't quote me on that, I could be wrong. The same 33% Accuracy would put you up to only a 74.5% final chance of hitting.
Adding a 10% To Hit bonus turns that 75% into an 85% in the first case, and the 56% into a 66% in the second case.

Many thanks, all. That's exactly what I wanted to know, and the links provided to elsewhere for even more detail were much appreciated, too.
Can someone explain the difference between ACC Mods and To-Hit Mods? I know they are different, but since I don't know ecactly how, it's hard to choose between a +ACC or a To Hit Bonus, or the reverse if talking about a debuff.
