He's wearing a meat suit, he can actually get out of it when he needs to.
Although not canon, the old old CoH Comics represented him as a normal human and all that extra bulk is some sort of horrific suit that he wears to do battle with heroes...
Sort of like Iron Man... except... made out of... corpses... eeeewwwww...
Okay... yeah... I can see that now. Disgusting all the more so though that he appears to be naked in that thing.

One of my tankers wears a meatsuit for formal, and some social, events, but it's more like synthetic flesh to provide a human appearance.

Dr. Vahzilok as we all know is effectively Dr. Frankenstein gone even further off the deep end. Instead of making one grotesque animated monstrosity, he has a whole army of them and a bunch of likewise mentally bent doctors to look after them. The thing is though... how does Dr. Vahzilok perform surgery these days? He has replaced his delicate surgeon's hands with a big, huge arm cannon with a axe-like bayonet, and a big, clumsy, beefy hand on the other arm. The guy's effectively crippled his manual dexterity. The guy would likely crush a china cup if he tried drinking tea now. This man cannot do the things he used to be able to do with the crazy behemoth of flesh body he's given himself. Does he just supervise surgeries now and silently weep at how stupid he was to cut off his hands?
