Unless you know something I don't please remove Pariah anchorite from the Villain list. Mapmaster has already stated that he mistakenly marked that as confirmed on the Vidiotmaps table, and no one has come forward with a method for a villain to get it.
You can add Coralax Blue Hybrid to the Gladiator section too.
And Crystal Keeper to the accomplishment section.

Nice updated list.....not to be nitpicky here but it was confusing at first 'cause I thought the checks were on the left instead of the right. Just because it's so close to the other badge. I think if you made it like ___Badge it'd be easier to read. But just a thought

Achievements:
Coldhearted: Killing CoT cold demons (Hoardlings)
Monkeywrencher: Killing Zeus titans (in Warburg)
Tracer: Killing Tsoo Sorcerers (Must be under lvl 29)
Deathless, Undying: Both are debt repayment badges
Role Model, Epitome: Both are mentoring badges.
Gladiator:
Bladegrass: Kill Grassy devouring earth
Caliban: Came with Regenerator
Cog: Came with Gearsmasher
Crane Enforcer: Came with Tracer
Fortunata Seer: Not sure, I think just killing Fortunata Seers
Crey Power Tank: Killing Crey Tanks (Found in North Nevra)
Shivan Destroyer: Came with Irridated, I think.
Accomplishment:
Leviathan: Sharkshead SF, badge is now fixed.
You may want to remove or somehow note Member of the Scorpion clan, it was being incorrectly rewarded for the SG computer SF and cannot now be gotten, as far as I know.

Thats not entirely correct. It's for both hordlings and Hellfrost. It says cold demons, not specificly Hellfrosts.
After making my post, I happend to notice the badge no longer says 250 Hellfrosts 
when I got it on my MM, it did specify, oh well. Easier to get now I guess
