You can activate hasten while still having the buff but it will not stack. The refreshed buff will replace the older one.
that said I frequently notice the icon APPEARS to show that some powers are recharged when in fact they aren't and so you might see that hasten appears clickable but it isn't actually ready yet.

I don't think you get double hasten, but I believe refreshing it before the previous one ends prevents the endurance crash.

No, it doesn't. There is ONLY ONE crash that can be avoided by making the power that causes it perma, and that is the -Defense crash in Rage.

When Domination ends - all accrued progress on building up domination is lost - thats a crash. You have to build it back up again irrespective of whether the recharge timer has passed.
When Practiced Brawler ends you can fire it off again as soon as it has recharged nothing else needs to build up.
