I haven't tested it myself, but others have said that choice for going to primal earth counts.
To be safe you might want to copy it to the test server and try there before doing it on live. However at 22, you can't get any more missions at this point, and all the moral choices are solo-only arcs, so you can't "go back".
Even if the hero/villain choice to Primal Earth doesn't count, I'd expect you will be able to get it filled in Issue 19 and beyond.

You should also have done the Resistance Warden arcs. At 14, my Loyalist Scrapper already has 4/5 done, and is closing in on another 2 more choices. Would have been 3 more, but I inadvertantly fell victim to the Interrogator Kang/Doctor Steffard trap. >.<

I couldnt figure out how to get the Resistance arcs going. I have other characters in Praetoria at lower levels, so if you could let me know how, that would be great.
Quick update for those who were in a similar situation. The final choice (Hero/Villain) does indeed give you your 5th mark for this badge (Moral High Ground).

In Nova, talk to Robert Flores. After you finish his arc, he'll intro you to Tunnel Rat, who gives you Doctor Arvin. He gives you a Morality Choice at the end of his arc.
In IC, talk to Jessica Flores. At the end of her arc, she'll intro you to Luke Larson, who sends you to Doctor Steffard, who sends you to Seer 1381. She gives a Moriality Choice at the end of her arc.
Not exactly sure who to talk to in Neut yet. Haven't made it there.
You can find Robert on the lower walkway that follows the moat around the Magisterium. He's on the East side of the Magisterium. Jessica is behind the TPN building.

One thing you have to worry about, however, is the Interrogator Kang/Doctor Steffard trap I mentioned above. If you complete Kang's arc before you finish Luke Larson's arc, Steffard won't be there. >.<

Actually, here's a question. Shouldn't the choice made if you run through the tutorial be counting as one of the 5 choices?
I was just wondering since I got a toon ready to lvl to 20 and she has the mission to go to the portal (I ran a straight Loyalist Powers) and she only has three and I know the fourth is when you chose where you want to go.
I made two toons on test and ran them quickly through the tutorial (one Loyalist and one Resistance), and neither got credit for the choice from it.
Is this WAI? If it is, then it's a bit misleading since it is a choice you make in the during your characters play.
Huh. I could swear that my tutorial choice counted on my char that got it. She did 2 in the first tier, 1 in the second, and 1 in the third... oops. Right. The hero/villain choice. Nevermind...

So, I've completed both arcs for Power and Responsibility, and now have 4 out of 5 for the badge... where's the 5th? Do I have to complete one of the Resistance arcs? Or do I get the 5th for deciding where to go on Primal Earth?
I've already levelled way past the max mission levels (22 and a third at the moment), so I dont know if I'd be able to go back and pick up any resistance missions, and I'd hate to leave that bar unfilled.
