What archetype were you trying to respec? I recall there used to be a bug with Kheldians where the Nova and Dwarf powers were available early for slotting. That is, if you slotted them before you actually picked them up, the respec would fail. I don't recall if this was fixed.

Check your Netgraph, the one time I had a failed respec my Netgraph was showing lots of red spikes. I believe the failure of a respec when your connection is suboptimal is to prevent database corruption.

Ice brutes in CoV beta lost all their powers when those powersets were removed from them. They had brawl, and sprint.

I have encountered this problem.
DO NOT slot any of your old enhancements, and it should go through.
Otherwise, switch to an alternate build.
I ended up respeccing once I left Praetoria, so I can't be sure I can narrow down what's at fault.

1) you still have to lvl up the remaining lvls before you can respec (example, i was lvl 48, but only levelled up to lvl 41 on my 2nd build)
after i levelled up the remaining levels it was successfully
2)not sure this is a possibility, changing builds and trying to respec still within the 15 Min wait
